Description

The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) intends to negotiate a contract with the Gila River Health Care Corporation to develop and implement a healthcare enterprise information system, a community-based diabetes care coordination program, and a community-based primary, secondary and tertiary prevention program for diabetes on the Gila River Indian Community in Phoenix, Arizona. The authority for this sole source action is 41 USC 253 (c)(1), as set forth in FAR 6.302-1. This organization has the prerequisite knowledge, experience, and facilities for the completion of this proposed acquisition by reason of its preliminary and current work on this project.
