Description

This is a combined solicitation/synopsis for commercial items, prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This notice constitutes the only solicitation; a written solicitation will not be issued. The solicitation, No. 263-01-P(BA)-0050, is issued as a Request for Proposal (RFP). The solicitation/contract will include all applicable provisions and clauses in effect through FAC 97-20. The National Institutes of Health (NIH) in Bethesda, Maryland intends to acquire, by a fixed price indefinite delivery, requirements type contract, on a sole-source basis, disposable labwear. The labwear shall meet the following specifications: Specifications For Items Number 1& 6: 1. Shall be a disposable shoecover. 2. Shall be constructed of 1.25 oz Polypropylene. 3. Shall be light-blue in color and fit shoe sizes 10 2 through 15. 4. Shall be packaged 150 pair per case. 5. Shall deliver Section B Item Number 1 to Bldg. 36. Platform (NIH Campus). 6. Shall deliver Section B Item Number 6 to Bldg. 10, Rm 5C127. Specifications For Items Number 2& 5: 7. Shall be a labcoat constructed of 1.5 oz. Polypropylene. 8. The labcoat shall be constructed with knit cuffs and collar. 9. Shall be white in color with pockets and snap front closure. 10. Shall be packaged 30 per case. 11. Shall be available in sizes small through extra large. 12. Shall deliver Section B Item Number 2 to Bldg. 36. Platform (NIH Campus). 13. Shall deliver Section B Item Number 5 to Bldg. 10, Rm 5C127. Specifications For Items Number 3& 7: 14. Shall be disposable shoecovers. 15. Shall be constructed of 2.24 ml. polyethylene. 16. Shall be blue in color and fit shoe sizes up to size 14. 17. Shall be packaged 50 pair per box and 200 pair per case. 18. Shall deliver Section B Item Number 3 to Bldg. 36. Platform (NIH Campus). 19. Shall deliver Section B Item Number 7 to Bldg. 10, Rm 5C127. Specifications For Items Number 4& 8: 20. Shall be a spun-poly non-woven bouffant style lab cap. 21. Shall measure 21 inches. 22. Shall be available in white or blue. 23. Shall be packaged 100 caps per disposable box, 5 disposable boxes per case (500 caps per case. 24. Shall deliver Section B Item Number 4 to Bldg. 36. Platform (NIH Campus). 25. Shall deliver Section B Item Number 8 to Bldg. 10, Rm 5C127. Additional Specifications 1 Bidder shall provide a copy of their standard Quality Control Program and report associated with the products requested in the Request for Bids with their bids. 2. The Contractor shall provide at no additional cost to the Government consultative services to the National Institutes of Health concerning proper application and use of garments in end use environment. The Contractor shall be able to assure the NIH that proper garments are being used in potential infectious disease and chemical exposure environments. Contractor shall be available for in-service training at specified intervals. 3. The Contractor shall accept returned goods that are unacceptable at no fault of the Government with full credit refund and no return shipping charges or re-stocking fees. Replacement products shall be supplied with in 24 hours of initial notice that defective products exists. 4. The Contractor shall respond to end user product problems within a 24-hour time frame. 5. Products manufactured of recycled materials are UNACCEPTABLE and shall not be provided under this contract. It is the Government's belief that these requirements may only be met by Life Science Products, Inc. Notice of Small Business Set-aside: This is not a formal solicitation; however, participation shall be limited to small business concerns demonstrating the capability to accomplish the above; standard industrial classification code: 5047; small business standard size: 30. Small business concerns that respond to this notice must furnish concise responses directed specifically to the requirements mentioned above. Small business concerns demonstrating the capability to accomplish the above shall provide pertinent information in sufficient detail to demonstrate their ability to perform. Information furnished shall include enough material to perform a proper evaluation. Respondents will not be notified of the evaluation results. Information provided must demonstrate the contractor's ability to meet the specifications described herein. Award will be made to the technically acceptable low offer. A completed copy of the provision FAR provision 52.212-1, A completed copy of the provision 52-212-3, Offeror Representatives and Certifications- Commercial Items, is required as part of the offer. The clause 52.212-4 Contract Terms and Conditions- Commercial Items applies to this acquisition .The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes Or Executive Orders-Commercial Items applies to this acquisition. Sources demonstrating the capability to accomplish the above shall provide pertinent information that is sufficient in detail to demonstrate their ability to perform within 15 days of this notice. A determination by the Government not to compete this proposed contract based upon the responses to the notice is solely within the discretion of the Government. The proposed contract action is for supplies or services for which the Government intends to solicit and negotiate under the authority of FAR 6.302. NOTE: NUMBERED Note 1 AND 22.
