- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under simplified acquisition procedures in accordance with FAR Part 13. This announcement constitutes the only Request for Quotation (RFQ) and a written solicitation WILL NOT be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-19. RFQ 600-109-00 should be referenced on all proposals. The North American Industrial Classification System (NAICS) code for this procurement is 621111 and the small business size-standard is gross annual receipts NTE $5.0 million. This procurement is unrestricted. The VA Southern Nevada Healthcare System, Las Vegas, Nevada has a need for a Locum Tenens Board Certified Orthopedic Surgeon (candidate(s) may be a Board Eligible Physician if within four (4) years of postgraduate residency) to provide non-operative Orthopedic services at the Department of Veterans Affairs Addeliar D. Guy III Ambulatory Care Center, 1700 Vegas Drive, Las Vegas, Nevada 89106. Physician shall provide coverage Monday through Friday, 8:00 a.m. 5:00 p.m., excluding Federal Holidays. Physician(s) must have Basic Life Saving (BLS) certification. Physician shall not be required, except in case of emergency, to furnish such services on a National Holiday or during off-duty hours. Physician(s) shall perform the following administrative and clinical services at the facility: (1) Conduct eight orthopedic outpatient clinics each week. (2) See, evaluate, and develop treatment plan for all patients seen in the orthopedic clinic. (3) Conduct two compensation and pension clinics each week per VBA protocol. (4) All clinics shall run from 8:00 a.m. to noon and from 1:00 p.m. to 5:00 p.m. (5) See, evaluate, and treat urgent non-operative orthopedic problems that present to the Ambulatory Care Center during administrative hours; e.g., 8:00 a.m. to 5:00 p.m. (6) Utilize Computerized Medical Record System (CPRS) to document medical care. (7) Attend administrative and educational meetings as scheduled. These meetings will be held in lieu of clinic time. Offerors shall submit the following with their proposal: pricing schedule; curriculum vitae, medical license (from a State, Territory, or Commonwealth of the United States or the District of Columbia), evidence of board certification, evidence of ACLS/BLS Certification and DEA certificate of proposed physician(s); and evidence of company's Indemnification and Medical Liability Insurance. Offerors may propose more than one candidate. The Contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the Contractor shall provide the following: Worker's Compensation Insurance, Professional Liability Insurance, Health Examinations (to include current TB Test results), Income Tax Withholding, and Social Security Payments.Offerors shall provide a completed set of representations and certifications with their offer found at FAR 52.212-3, Offeror Representation and Certifications-Commercial Items. Provisions and clauses found at 52.212-1 Instructions to Offerors Commercial Items, 52.212-4 Contract Terms and Conditions Commercial Items, and 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, apply. The following FAR and VAAR clauses and provisions are added by addendum: 52.224-1 Privacy Act Notification; 52.224-2 Privacy Act; 852.237-7 Indemnification and Medical Liability Insurance; 852.270-4 Commercial Advertising; and 852.271-70 Services Provided Beneficiaries. The duration of required services is a minimum of six (6) months up to a maximum of one (1) year. Proposals will be accepted immediately, but no later than September 19, 2000. The anticipated starting date for these physician services shall be on or around October 2, 2000.
