Description

Provide Community Based Outpatient Clinic Services for beneficiaries of the Department of Veterans Affirs VA SLC Healthcare System. The Clinic must be in Richfield, Utah. The contractor shall provide for the delivery and management of continuous primary and preventive medical care for eligible veterans. The contractor shall provide health care clinical providers, medical facilities, medical equipment, supplies, imaging services, and all administrative functions to fulfill the level of care and range of services as per the terms and conditions of the contract. Services will be at a capitated rate and the price will be an all inclusive price. The resulting contract will be a base year with two one year options to renew the contract. The contract will be a fixed price commercial contract using FAR Part 12 & Part 15. NAICSC Code -- 621498 7.5 Million The solicitation will be issued approximately December 20, 2000.
