Description

The Contractor shall provide all necessary personnel, supplies, medications, equipment, facilities, and liability insurance to provide a complete ambulatory healthcare services with all ancillary and support services to Native Americans residing within defined towns/communities of Oljato, Monument Valley, Mexican Hat, and Goulding, Utah. Anticipated award will be for a twelve month period with two (2) one-year options. Interested offerors should submit their request for a copy of RFPM01-0005/PD.
