Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with 38 U.S.C. 8153 Expanded Sharing Authority and the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation Number RFQ 541-027-01 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-20. This is a competitive procurement under the North American Industry Classification System (NAICS) Code 622110 and Small Business Size Standard of $5.0 Million per annum. The Louis B. Stokes Cleveland Dept. of Veterans Affairs Medical Center intends to contract for the services of One Full Time Employee Equivalent (FTEE) Physician to provide Primary Care Physician Services to and at the Youngstown Community Based Outpatient Clinic (CBOC). Services shall be for a period of 12 months from date award with one 12-month option for renewal in accordance with the terms, conditions, and schedule contained herein. Offerors should thoroughly review the specifications and be familiar with the area of coverage prior to submitting their offer in order to be fully aware of the services required. Failure to do so will not relieve the offer from performing in accordance with the strict intent and meaning of the specifications without additional cost to the Government. Description of Service: Contractor shall provide to and at the Youngstown CBOC, located at 2031 Belmont Avenue, Youngstown, OH 44505, One FTEE Primary Care Physician 40 hours per week, Monday through Friday, between the hours of 8:00 a.m. through 5:00 p.m., excluding Federal holidays. Contractor will be required to perform an estimated quantity of 2000 hours per year. Offerors price proposal must provide an hourly rate and total cost for both the base year and option year 1. Definitions and Acronyms: a) CBOC Community Based Outpatient Clinic: b) FTEE Full Time Employee Equivalent; c) Primary Care Is the provision of integrated, accessible health care services by clinicians who are accountable for addressing a majority of personal health care needs, developing a sustained partnership with patients, and practicing in the context of family and community; d) Workday Shall be defined as eight hours per day, between the hours of 8:00 a.m. through 5:00 p.m., Monday through Friday, excluding Federal holidays. Requirements: a. The contract provider shall provide One FTEE Primary Care Physician under the terms and conditions of this contract, to perform complete primary care services at the Youngstown CBOC, Monday through Friday, between the hours of 8:00 a.m. through 5:00 p.m., excluding Federal holidays. b. The Contract providers shall be responsible for (i) for management of patients with chronic medical conditions, (ii) exhibit professional conduct among staff and patients, (iii) provide legible documentation of patient care, (iv) see an acceptable workload, (v) be in compliance with VA preventive health care guidelines, (vi) provide initial stabilization and triage of any emergent patients, and (vii) use appropriate judgement in referring patients to specialists at the Louis Stokes Cleveland Department of Veterans Affairs Medical Center, Brecksville/Wade Park, as needed. c. Contractor personnel providing services under this contract shall not exceed three (2) physicians. d. The contractor and/or contract provider shall be required to provide proof of Indemnification and Medical Liability Insurance, in accordance with the requirements of Veterans Administration Acquisition Regulation (VAAR) "852.237-7 Indemnification and Medical Liability Insurance", in the amount of not less than $1,000,000.00. Stated proof of indemnification and medical liability insurance shall be provided to the Contracting Officer prior to providing any services at the Youngstown CBOC. Hours of Service: a. Contractor shall provide one qualified physician, on-site at the Louis Stokes Cleveland VA CBOC, Youngstown, Ohio, between the hours of 8:00 a.m. until 5:00 p.m., Monday through Friday, excluding Federal holidays. b. The following terms have the following meanings: (1) Work hours: 8:00 a.m. through 5:00 p.m., Monday through Friday, excluding Federal holidays. (2) Nonwork hours: 5:01 p.m. through 7:59 a.m., Monday through Friday, and 24 hours on weekends and Federal holidays. For purposes of this agreement a calendar day shall mean from 12:00 a.m. through 11:59 p.m. (3) The 10 Federal Holidays observed by the Government are as follows: New Year's Day; Dr. Martin Luther King Jr's Birthday; Presidents Day; Memorial Day; Independence Day; Labor Day; Columbus Day; Veteran's Day; Thanksgiving; and Christmas. Any other day specifically declared by the President of the United States to be a national holiday. Privileging/Credentialing Procedures: a. The contract physician providing services to patients of the Louis Stokes Cleveland VA CBOC, Youngstown, Ohio, shall have proof of a medical license from a State, Territory, or Commonwealth of the United States or District of Columbia and a certificate from a recognized American Board. The Contracting Officer shall forward Credentials to the VA Professional Standards Board for review and recommendation to the VA Medical Center Director or his/her designee, for approval or disapproval of granting privileges. (i) Contractor shall provide sufficient verifiable credentials to warrant the granting of clinical privileges by the VA Medical Center Director. (ii) The Medical Center Director or his/her designee must provide each Contract Primary Care Physician privileges, prior to the contractor providing any service under this contract. (iii) Each applicant seeking clinical privileges shall request in writing the specific clinical privileges desired on the privileging document for approval by the Medical Executive Committee. Contractor must submit appropriate appointment information as outlined in the "Medical Staff Bylaws and Rules", "Articles IV, Section 2 Application Procedures" and "Article V, Section 2, Process and Requirements for Requesting Clinical Privileges" (a copy of the Medical Staff Bylaws and Rules are available upon request). b. The contractor shall allow a minimum of 60 calendar days for review of credentials by the VA Professional Standards Board and the VA Medical Center Director or his/her designee. Period required for review of credentials shall be at no additional cost to the Government. Contact providers must furnish evidence of all the above criteria and must provide references of good clinical work and ethical conduct. The services specified herein may be changed by written modification to the contract. The modification will be prepared by the VA Contracting Officer and prior to becoming effective, shall be approved by the VA Medical Center Director or his/her designee. Other necessary personnel for the operation of the services contracted for at VA will be provided by the VA at levels mutually agreed upon which are compatible with the safety of the patient and personnel and with quality medical care programming. The services to be performed by the contractor shall be performed in accordance with VA policies and procedures and the regulations of the medical staff-by-laws of the VA facility. The services to be performed by the contractor shall be under the direction of the Chief of Staff or his/her designee, VA Medical Center, Cleveland, Ohio. The contractor shall be responsible for providing: worker's compensation; professional liability insurance; health examinations; income tax withholding; and social security payments. The parties agree that the contractor shall not be considered a VA employee for any purpose. A sign-in-sign-out log shall be maintained in the office of the secretary, CBOC Director. Contractor personnel shall sign-in upon arrival at the CBOC, and sign-out upon departure. Review of the sign-in/sign-out shall be performed by the timekeeper for the CBOC, prior to payment of invoices to ensure hours requested are actual hours worked. Failure of contractor personnel to adhere to the above sign-in/sign-out requirements, may have adverse effect on the processing of invoices and delay payments. Also, failure to comply with the strict intent of the sign-in/sign-out procedures may be considered a breech of the contract and may be considered as grounds for default. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial items applies to this acquisition with the following changes, items (e) and (h) of the standard clause are deleted. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial items, with their offer (a copy of the provision may be obtained at the following Internet Web Site http://www.arnet.gov/far/current/pdf/toc.html). The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses: 52.203-6, 52.219-8, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-3, 52.225-13, 52.225-16 and 52.232-34. The following standard FAR and VAAR clauses are incorporated and apply to this acquisition: 52.217-8, 52.217-9, which provides that the Government may extend the term of the contract within 60 calendar days; provided that it provides preliminary notice 60 calendar days prior to the expiration date, and that services shall not exceed two years, 52.232-18, 52.232-19 (which provides that funds are not authorized past September 30 of any fiscal year, unless written authorization is received from the Contracting Officer), 52.233-2 (which provides that protests filed with the General Accounting Office (GAO) shall be served on the Contracting Officer 90C(B), VA Medical Center, 10000 Brecksville Road, Brecksville, OH 44141), 52.233-3, 852.233-70, 852.233-71, 852.237-7 (which requires submission of Indemnification and Medical Liability Insurance in an amount consistent with the prevailing rates for the state of Ohio ($1,000,000)), 852.270-1, 852.270-4, and the provision at 852.219-70 (fill-in provision which may be obtained at the following Internet Web Site http://152.128.2.7/oa&mm/vaar/vaar852.htm) and 852.270-1.
