Description

This notice is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Written Quotes are being requested and a written solicitation will not be issued. This procurement is being conducted under Simplified Acquisition Procedures (SAP). The reference number for this procurement is RFQ 670-10-01 and incorporates the clauses and provisions that are in effect through Federal Acquisition Circular 97-20. Quotes should be for the following items: TOPCON or equal 800 x 600 FA/Color Capture IMAGEnet System to include Pentium III or equal 550MHz Based Compaq 6400Ep Series Small Business Computer, 2 ea. 9.1 Gig SCSI 2HDD (or larger)+ 64 MB RAM, PAL 800x600 Image Capture /Display, Color Capture Display Card, 19" Color UVGA Monitor, High Speed Modem (56 KPS), Windows NT + IMAGEnet 2000[32bit] Windows Software, Communication and Utility Software, CD Archiving (Write 8X/Read 20X), CD Reader 30X, Interface to Retinal Camera (TRC-50EX), 3 chip Sony PAL 950P Color CCD Camera, System table and necessary wiring, battery back up & line conditioner. Patient arm rest support. MPS/PDT Rings. Lens Cleaning Kit. TRC-50EX or Equal retinal camera system to include 1TRC-50EX retinal camera, 2 each MT-10 camera backs, 1 ea filter set for FA, Internal fixation, AIT-11 electric Table, Double Table Top. Kodak XLS 8670 (dye sub color/ B&W ) printer [high end] includes 3' SCSI cable, 1 paper Pak (100 sheets), 1 ribbon (150 images), and super print driver for 8670. Networked Laser review station to include Pentium III 500 MHz Based Compaq (Prosignia) PC, 6.4 Gigabyte HDD + 64 MB RAM, 4Mbyte Video controller card(AGP-3D graphics adapter), 3.5" disk drive, 17" color SVGA monitor, IMAGEnet 2000 review station for windows NT, CD Reader (32x), Network card and cabling (up to 150'). Installation and TWO day Training. Equipment to be delivered within 45 days after receipt of order. Contractor shall include warranty information. Quotes received shall specify FOB DESTINATION. VAAR Clause 852.210-77 Brand name or equal applies to this solicitation. NASICS code is 339112 and small business size is 500 employees. The provisions at FAR 52.212-1 Instruction to Offerors-Commercial Items and FAR 52.212-3 Offerors Representations and Certifications applies to this acquisition and a completed signed copy of this provision shall be submitted with the quote. The Representations and Certifications may be obtained via the Internet at: http://www.arnet.gov. The provisions at FAR 52.212-2 Evaluation-Commercial Items also applies to this acquisition. The following factors, in descending order of importance, will be used to evaluate quotes: (1)determination of product acceptability, (2) lowest priced quotations, (3) delivery schedule and (4) technical ability. Prior to acceptance of the goods or services provided under this contract it will be necessary for the government to conduct a technical acceptance inspection. This inspection will be completed within 10 calendar days of delivery of the goods or services provided under this contract. A written notice of award or acceptance of offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer shall result in a binding contract without further actions by either party. Offerors responding to this announcement shall submit a price for this system in accordance with FAR 52.212-1 to include: a completed copy of provision 52.212-3 (Offeror Representations and Certifications), a completed copy of VAAR 852.210-71 (Guarentee) and 852.219-70 (Veteran Owned Small Business) and, if bidding other than brand name, the manufacturer, model or catalog number, and specifications of the equal.
