Description

The Aberdeen Area Indian Health Service proposes to award a Firm Fixed-Price Non-Personal Healthcare Service Contract for a certified Optometrist to work three (3) days a week at the Rapid City PHS, IHS Hospital, 3200 Canyon Lake Drive, Rapid City, South Dakota 57702, Attn: Optometry Department. The period of performance will be for one (1) year from date of award. The proposed contractor shall comply with the medical staff fules, regulations and by laws related to clinical practicce as promulgated by the Clinical Director of the Rapid City PHS, IHS Hospital and the National Standards of the Optometrical specialty. This service will be performed in accordance to the Statement of Work as included in the solicitation packet. Request for solicitation packet RFQ-01-020 shall be made in writing or telefax at (605) 226-7669 and will be made available approximately 15 days after publication of this announcement.
