Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. Solicitation number: RFP 504-01-007. This solicitation document and incorporated provisions are those in effect through FAC 97-20. Contractor shall supply or sub-contract for all services and materials associated with "Microbial Remediation at the VA Medical Center, Amarillo, Texas. Broken steam pipe lead to area becoming covered with mold. Repair of defective pipe has been conducted. Presence of fungi on building materials was identified with visual assessment. Contamination is considered extensive. HVAC Systems are not affected. Exhaust fan and heaters in the area have been shut down. Containment of the affected area has been completed using plastic sheeting. Bulk/surface sampling results identified Aspergillus versicolor and/or Stachybotrys chartarum. The simplest and most expedient remediation that can be safely and reasonably removed by proper standards for fungal contamination should be used. Requirements: 1. Removal and replacement with like materials for approximately 744 square feet of 5/8" gypsum board; 75 linear feet of fire caulking at the gypsum board wall and 744 square feet of fiberglass insulation. 2. Removal and replacement with like materials pipe insulation (1 1/2" 3" diameter) from approximately 706 linear feet of piping as follows: HPS -- 3" pipe, 114 l.f. of 2 1/2" thick insulation. PC -- 1 1/2" pipe, 100 l.f. of 3/4" thick insulation. CH -- 4" pipe, 100 l.f. of 2" thick insulation CHR -- 4" pipe, 100 l.f. of 2" thick insulation HWH -- 2 1/2 " pipe, 118 l.f. of 1 1/2" thick insulation HWH -- 3/4" pipe, 46 l.f. of 1" thick insulation HWHR -- 1 1/4" pipe, 128 l.f. of 1 1/2" thick insulation 706 linear feet of piping requiring re-labeling approximately every 20 l.f. If penetration at pan is fireproof sealed, remove pipe insulation only to underside of pan. If penetration is not fireproof sealed at underside of pan, remove pipe insulation to approximately 1" above top of pan. 3. Treatment or removal of fungal contamination from approximately 175 sq. ft. of fireproofing on steel support beams. Replacement/supplementation of fireproofing to maintain integrity of steel support beams. 4. Manual Cleaning (Wipe) of effected areas, including galvanized tin ceiling/floor pan surfaces and concrete walls using appropriate biocide treatment. 5. Heavy mil plastic sheeting be used to cover ground (gravel) and collect debris. 6. MSDS for any anti-microbial applications or biocide treatment to be provided by contractor. VA reserves the right to review and approve biocide selected. 7. Provide exhaust fan with HEPA filter to generate negative air pressure in affected area for remediation period. 8. Provide for third party background (initial) and clearance (post remediation) air monitoring for fungi utilizing an AIHA accredited laboratory (Environmental Microbiology Laboratory Accreditation Program, EMLAP) and must participate in quarterly proficiency testing (EMPAT). Testing to be conducted in affected area, the non-contaminated side of the barrier, and of outside air. Third party visual inspection should be accomplished of internal surfaces during project. 9. Provide proper isolation barriers for containment area to ensure fungal contamination is not released into adjacent uncontaminated occupied areas of the building. 10. PPE for workers, appropriate for given contaminants encountered during the project, will be supplied by contractor. Space is available for decontamination room. 11. Provide appropriate documentation (waste manifest) for waste disposal in compliance with local, state, and federal requirements and in a method satisfactory to owners. 12. Project final written report will be provided. 13. Contractor will comply with applicable OSHA and EPA Regulations. 14. Provide remediation following New York City Department of Health, Bureau of Environmental and Occupational Disease Epidemiology, "Guidelines on Assessment and Remediation of Fungi in Indoor Environments" located at web site www.ci.nyc.ny.us/html/doh/html/epi/moldrpt1.html Provision 52.212-1, Instructions to Offerors Commercial Item applies to this solicitation. Provision 52.212-2, Evaluation -- Commercial Items is included and the following evaluation criteria will be used. 1. Technical, Past Performance, and Pricing. Offerors will only be considered that have mold abatement experience. A detailed schedule of work must accompany proposal. Offeror must include a copy of the provision 52.212-3, Offeror Representations and Certifications Commercial Items. Clause 52.212-4, Contract Terms and Conditions commercial Items applies, no addendum added. Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies which includes the Service Contract Act. Site visits can be requested.
