MODIFICATION TO PREVIOUS NOTICE: The following paragraphs of the Statement of Work for Plastic Bead Media have been changed. This change is effective 11 Dec 2000. Remove and replace the following corresponding paragraphs: 5.5.1.2 All storing, handling, manufacturing processes, and recycling of the SPENT MATERIAL shall be performed at the facilities of the prime contractor. 7.0 PERMITTING The contractor shall obtain all necessary licenses, permits, and approvals required by any regulating agency with regard to the obligations and/or work to be performed in this Statement of Work. The contractor, at time of bid, shall possess all necessary licenses and permits as required by any regulating agency with regards to the obligations and/or work to be performed in the SOW. The contractor, at time of bid, shall be approved for their recycling program by the environmental agency of the state in which the material to be recycled will be shipped from and the environmental agency(ies) of the state(s) in which the recycling process takes place. All such documentation shall be part of the contractors bid package. A firm-fixed price purchase order will be issued under the Simplified Acquisition Procedures using FAR Part 12 and FAR Part 13.5, Test Procedures for Certain Commercial Items. Please submit signed and dated offer to 3 CONS/LGCB, 10480 22nd Street Elmendorf AFB Alaska 99506-2500 at/or before 1030 am Alaska Time, 12 DEC 2000. Quotations shall be signed, dated and submitted on Company letterhead or quotation forms.
