Description
This is a combined synopsis/solicitation for commercial items for the Department of Veterans Affairs and is prepared in accordance with the format in Subpart 12.6, as supplemented with additional information provided in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will NOT be issued. Soliciation No. RFQ 512(B)-160-01 is issued as a Request For Quote document and incorporated provisions and clauses are those in effect through FAC 97-20. The NAIC is 421450 with Size Standard $100 million. The VA Maryland Health Care System intends to purchase one (1) Industrial Acoustics Company Model 1200-A-CT Series Sound Suite of double-wall construction manufactured by Industrial Acoustics Company, Inc., 1160 Commerce Avenue, Bronx, NY 10462. The suite must be in accordance with ALL specifications for IAC Medical/Life Sciences Examination and Research Rooms. The suite consists of a 400-A-CT 2-room suite contained with an outer enclosure of 4-inch (102mm) modular panels and flooring is required on both sides. Two optional items are required: (1) Power filters and (2) a 6-Outlet "plug'in" power strip. The Veterans Administration will provide the air supply, but supplier will provide the HVAC adapters to the sound booth. This procurement shall include shipping and installation at the VA Outpatient addition located at 3900 Loch Raven Boulevard, Baltimore, MD 21218; System 20-lookup software to allow VA supplied workstations to view previously stored audiometric data. Reports shall be printed on standard forms that include audiometric, impedance, comments, hearing and data page, abr and eng summary data page and real ear capture page. The MANDATORY delivery and installation date shall be no later than July 1, 2001. (The exact date shall be coordinated with the VA.) The following incorporated clauses and provisions are applicable to this acquisition: 52.212-1, Instructions to Offeror -- Commercial Items, includes addenda, 52.233-2; Offerors are to include a completed copy of the FAR provision 52.212-3 (including VAAR 852.219-70), with their offer. Also, included, 52.212-4, Contract Terms and Conditions -- Commercial Items, includes addenda, 852.211-77; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, includes 52.219-8, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, and 52.225-3.
