- DESC:This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation number 644-14-01 is a Request for Quotation and incorporates provisions and clauses in effect through Federal Acquisition Circular 97-18 . The NAICSC code is 339112. This procurement is unrestricted. Clauses and Provisions from the Federal Acquisition Regulation (FAR) may be obtained online at http://www.arnet.gov/far, and Veteran's Administration Acquisition Regulation (VAAR) http://www.va.gov/oa/&mm/vaar. The Carl T. Hayden VA Medical Center wishes to purchase one (1) new or used IntraCoronary Ultrasound In-Vision Imaging System with in-line 3d Resolve/ Digital/Longitudinal Reconstruction Option; Intravascular Chromoflow/Color Flow Option; EndoVascular AAA Option. Offeror must provide delivery and installation. System must be digital formatted and DICOM compatible and have a minimum of one year warranty, parts,labor,on-site repair service and all software upgrades provided free during the period of the warranty. Disposable supplies must be available for the system. If used equipment is offered, all cited Options must be provided and the equipment must not be more than eight (8) months old or have more than 100 hours of use on the equipment. If used equipment is offered, all other specifications cited in this solicitation must be provided. The equipment must perform without the need for a dye load. The patient interface module must have forward and backward capability with at least a 25 foot cable. Central processor must have a 32 bit, IVOS operating system, image display must be 17" diagonal color with 0.25 super fine pitch. Image display and processing must provide at least multiple images for ultrasound or videoloop stored untrasound frames, fluroro; ultrasound or stored ultrasound frames, fluoro; ultrasound or videoloop stored ultrasound frames; ultrasound or stored ultrasound frames; fluoroscopy high and low line image diameters at 8mm, 10mm, 12mm, and 14mm. Image storage must provide 6 ultrasound freeze frames and 60 second videoloop. The processing functions must provide gain: 0-68dB in 2dB increments for five-64, 4-36 for Beta; Persist: 5 settings; Grayscale: 8 bit; 7 Grayscale Curves; Frame: Videoloop Frame Counter; Image Mode: 3 Presets, 2 Custom; Catheter: Auto Presets. The displayed graphics must provide at least: time, date, hospital, doctor, patient name and ID, alerts; and Multi-Menus; System Parameters Annotation: on image and surrounding area; Measurements: 4 distance, 2 area, % difference for distance and area, and EKG trace. The Operator Devices must provide at a minimum: TCG Controls: 5; Screen Menu Choice Keys: multi-menu selection; Dedicated Function Keys: machine controls; Precision Trackball: measurements, menu selection; a Full Sized Keyboard; and VCR with all VCR functions including recordable CD (removable media), image archiving, and read 4x-write 2x. Auxillary interfaces must have remote fluoroscopy, ethernet (thin co-ax), high level biophysical signals-2 channels.Must have 100,120, 200 or 230 VAC, with Frequency/Power of 50/60 Hz, 1500-1800 VA. Offers should include descriptive literature for all equipment and options. Delivery shall be made to the Carl T. Hayden VA Medical Center, 650 E. Indian School Road, Phoenix, AZ no later than 14 days after award. The evaluation criteria included in paragraph (a): The following factors shall be used to evaluate offers: ( 1) The technical capability of the item(s) offered to meet the Government Requirement and (2) Price, (3) Warranty, (4) Age of Equipment and Hours of Use.
