- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All responses to this request for proposal (RFP) must make reference to solicitation number F04693-01-R0004. SMC/PKOV is procuring and securing a contractor, who shall furnish all management, supervision, personnel, labor, tools equipment, transportation, materials, parts, and other items and services necessary to inspect, test, and maintain elevators at Los Angeles Air Force Base. The contractor shall provide proof of applicable state or local elevator licenses, for electrical or hydraulic type elevators, and that mechanics are certified through the National Elevator Industry Education Program. The duration of the contract period is base year (beginning 1 January 2001-30 September 2001) to include four option years (starting 1 October 2001-30 September 2005). All work shall be performed in accordance with the attached Statement of Work (SOW). The Wage Determination No. 1986-0431 Revision No. 23 is also attached. Both attachments will be posted on http://www.eps.gov. An informal site visit will be available upon request by calling Bella Hunter at 310-363-2188 for an appointment. All proposals shall include a quality control plan and copies of all current required licenses. The SOW details the requirements of the contract. To be eligible to receive an award resulting from this announcement, DFAR 252.204-7004, Required Central Contractor Registration, requires the contractor to be registered with the Central Contractor Registration (CCR) database, NO EXEMPTIONS. In order to register, apply through the internet at http://www.ccr2000.com. A Dun and Bradstreet (DUNS) must accompany your proposal. The proposal document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 97-20. The provision at 52.212-1, Instructions to Offerors- Commercial, applies to this acquisition and a statement regarding any addenda to the provision. FAR 52.212-2, Evaluation-Commercial Items, the Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered, with price the leading factor. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, applies to this acquisition and a completed copy of the provision should be provided with the proposal. This acquisition is a small business set aside. FAR 52.212-4, Contract Terms and Conditions-Commercial Items; applies to this acquisition and any statement regarding any addenda to the clause. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, including the following subclauses 52.222-3, 52.233-3, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, and 52.232-33, apply to this acquisition. Oral communication is not acceptable in response to this synopsis/solicitation. All interested vendors submit their proposals with requested items above by close of business on 19 December 2000.
