Description

The A/E shall completely design "Relocate GI, Pulmonary & Cardiology" Project which is located on the entire sixth floor of Building No.1, approximately 27,000 square feet, at the VA Medical Center, 650 E Indian School Road, Phoenix, AZ. Work shall include, but not limited to, demolition including asbestos abatement, complete removal of walls, ceiling, flooring to accept new clinics providing for treatment room/exam rooms, sleep lab, offices, storage, waiting rooms etc. All areas involved will be designed for mechanical, electrical, finishes, and be site ready for any new equipment to be installed. Services include field investigation, design, preparation of contract drawings and specifications, construction cost estimate and construction period services. All design shall be in accordance with NFPA, ADA, VA guidelines and directives, VA master specifications and all other applicable codes. A/E is responsible for site survey to evaluate existing conditions and provide coordination, review and consultation services through the completion of the project. A/E shall provide Professional Industrial Hygienist Services to provide specifications and drawings for abatement. Design Submissions are required at 30%, 60% and 100%. VA will provide the most current as-built drawings possible and other survey information as may be useful. Drawings shall be provided in AUTOCAD Version 14. Specifications shall be provided in Microsoft Word 97. During Construction Period Services the A/E shall review all contractor submittals (shop drawings, technical data), cost proposals, and RFI's, through the project construction duration. A/E shall witness acceptance tests where required and perform interim site visits/inspections as needed. A/E shall provide accurate As-Built Drawings. NAICS Code is 541330. This is not restricted to Small Business. The Construction Range is between $1,000,000.00 and $2,000,000.00. This is not a request for proposal. All interested, responsible and qualified business firms shall submit two (2) copies of their SF 254 and SF 255s to the attention of the Contracting Officer, to be received no later than CLOSE OF BUSINESS (4:30pm local time) on January 17, 2001.
