Description

This is a solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12 and 14 and Veterans Administration Acquisition Regulations as supplemented with additional information included in this notice. A written solicitation will be issued. Solicitation number 673-64-01 applies and is issued as a invitation for bid. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-20. The North American Industry Classification System (NAICS) code is 621910 and the business size standard is $5 million. Furnish BLS ambulance service for beneficiaries of the Orlando VA Healthcare Center (Clinic and Nursing Home Care Unit), 5201 Raymond Street, Orlando, Florida, to include Community-Based Outpatient Clinics (CBOCs) located at 209 San Carlos Avenue, Sanford, Florida 32771; and 201 Hilda Street, Kissimmee, Florida 34741 for the period February 1, 2001 through September 30, 2001. The Government may extend the term of this contact three (3) additional years. Qualified bidders shall meet all requirements of Federal, State, County or City codes, and licenses regarding operation of this type of service and shall provide proper documentation with the bid. The above due date should be considered "on or about". Only written or faxed requests received directly from the requestor no later than December 28, 2000 are acceptable.
