Description
The Defense Finance & Accounting Service-Indianapolis Center intends to issue a combined synopsis/request for proposal for commercial items in accordance with FAR, Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The Request for Quotation (RFQ) Number is MDA260-01-Q-0001 and should be referenced on all correspondence. The RFQ document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-19. This acquisition is unrestricted. The standard industrial classification code is 7991, annual receipts may not exceed $5.0 million. The line items, unit issue, quantity and description are as follows: 0001, 12 months, YMCA memberships for physical fitness facility services for approximately 90 active duty military and federal civilian personnel in accordance with the attached Statement of Work for the period February 15, 2001 through February 14, 2002 (Base Year); 0002, 12 months, YMCA memberships for physical fitness facility services for approximately 90 active duty military and federal civilian employees in accordance with attached Statement of Work for the period February 15, 2002 through February 14, 2003 (Option Year One). Both line items are NOT TO EXCEED without prior Contracting Officer approval. Statement of Work: Physical Fitness and Health and Wellness Extension Services for Military and Civilian Employees. Requirement: Access to full service physical fitness facilities for military and civilian personnel assigned to and working at the Defense Finance and Accounting Service Orlando (DFAS-OR), Orlando, Florida. Scope: The government anticipates: (a) Purchase of not more than 90 memberships for DFAS-OR military personnel; (b) Availability of corporate membership for DFAS-OR civilian personnel; (c) Memberships waive initial joining fee and discount 30% from the standard open market membership fee. Specifications: The contractor shall provide: (1) Access to physical fitness facilities, qualified instructors, formal health programs and services; (2) Dedicated access to gymnasium floor for approximately 90 people from 6:40a.m. until 7:30a.m. Monday, Wednesday and Friday for military/group physical fitness training; (3) Access to the physical fitness facility will be through individual membership cards issued to the fitness facility personnel; (4) The facility must be within a 15-minute driving distance from the Defense Finance and Accounting Service Orlando, 2500 Leahy Avenue, Orlando, Florida to reduce the amount of travel time for military members between the fitness facility and the work location. The facility must be easily accessible by employees before and after duty hours and during the normal business day for those individuals who opt to use the facility during flex hours, i.e. lunch hour; (5) Privileges of a regular membership to include unlimited access to any of the contractors facility locations; (6) Services to include, but not be limited to, use of an indoor gymnasium, community center or recreation center complex with indoor and outdoor swimming pools, cardiovascular equipment (minimum of 6 star masters), stationary (minimum of 24 Nautilus type machines) and free weights (to include dumbbells, bench press, etc.), workout room, showers and secured locker facilities and access to a 2-mile running track or trail. Facility to be available for use from 5:30a.m. until 9:00p.m. Monday through Friday, and on weekends. (7) Certified physical fitness instructions, health, wellness and educational programs; (8) Screening to ensure that the civilian employees enrolled in the DFAS program have completed the fitness assessment prior to issuing a corporate membership card; (9) Monthly billing to the DFAS-OR (Attn: DFAS-N/OR) 2500 Leahy Ave., Box 931100, Orlando, FL 32893-1100 for the military memberships; (10) Direct billing to the civilian employee for their corporate membership costs; (11) Optional family memberships and variety of family oriented activities are available at a discount of 30% from the standard open market membership fee. The Defense Finance and Accounting Service Orlando retains the right to terminate individual military memberships when members are transferred from DFAS-Orlando or are separated from the military service and reassign those memberships to new incoming military members. DFAS-Orlando also retains the right to reduce the total number of military memberships if downsizing occurs. Length of Contract: Initial one year plus one option year. Background: Commanders/Directors may use contracts for organizational memberships in and use of local commercial or municipal fitness facilities when military facilities are not available or adequate. The soldiers and airmen assigned to DFAS-OR no longer have the support of a military facility to conduct their required physical fitness training. Scientific studies indicate that individuals who maintain their participation in wellness programs increase self-efficacy, developing appropriate coping responses and extended periods in fitness activities. These views point out the potential importance of regular physical activity at moderate intensity toward the psychological health of the individual. Public health goals acknowledge the need for intervention strategies that can improve the physical activities of employees. End of Statement of Work. This acquisition is being procured under Simplified Acquisition Procedures. The Department of Defense requires all contractors to be registered in Central Contractor Registration (CCR) no later than May 31, 1998 to receive solicitations, contract awards or payments.
