This will be a firm-fixed price contract for labor, material, transportation and replacement parts in accordance with a full service maintenance agreement which encompasses scheduled and unscheduled service calls, remedial and preventive maintenance for the following items. Place of Performance/FOB: Robins AFB, GA. Period of Performance: Date of Award through 30 Sep 2001 with 2 annual one-year option years. Service calls shall be placed by Medical Maintenance personnel(phone 478-327-8269) or their designated representative. Items 0001/0002/0003 require Service with partial glassware coverage.Items 0005/0006/0007/0008/0009/0010 require Service without glassware coverage. Item 0004 requires Service with full glassware coverage. All line items require 2 Preventive maintenance Services each year. This solicitation is sole source to GE Medical System, W-443, 3000 N. Grandview, Waukesha, WI 53188. CBD Notes 22, 26 and Local WRALC Note 1 apply to solicitation. Approximate ISSUE DATE: 20 December 2000 Approximate RESPONSE TIME: 29 December 2000. Electronic procedures will be used for this solicitation. Justification: services (supplies) required are available from only one or a limited number of responsible sources and no other type of services or supplies will satisfy agency requirements.
