Description

The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) of the National Institutes of Health (NIH) requires a company to print and distribute the U.S. Renal Data System 2000 Annual Data Report from electronic files. The company shall produce the 800-page, 8-1/2"x11", textbook-type report from an electronic file (PC PageMaker) using a KBA/Planeta 64" press. The specifications for the printing job are as follows: 1) Copies -- 5,300; 2) Pages -- 800: 544 pages of text print black throughout, including 5 B&W graphics, 256 pages of text print four-color process throughout, including 482 color graphics; 3) Cover -- 4 pages, prints four-color process on one side; 4) Binding -- Perfect, 11" spine; 5) Paper Stock -- Cover 10pt C1S cover, Body -- 70 pound text gloss; 6) Final Trim Size -- 8-1/2"x11"; 7) Packaging -- Bulk pack carton for shipment to individuals on mailing list; 8) Proofs -- two (2) sets of bluelines and digital color halftone proofs are required. All responsible 8(a), small business, and other vendors that are capable of providing to the Government the services described above may request in writing or by FAX (301-480-4226) the Request for Quote (RFQ) from the Contracting Office prior to the closing date of this notice. It is anticipated that RFQ NIH-NIDDK-01-609 will be available for fifteen (15) calendar days after the publication date of this notice. Quotations will be due within ten (10) calendar days after the RFQ closes. The anticipated award date is January 31, 2001. Quotations must be submitted in writing to NIDDK Contracting Office, 6705 Rockledge Drive, Building One, Room 8048, Bethesda, Maryland 20817-7973, Attention: Patricia Haun. FAX quotations must contain the offerors name, address, and telephone and fax numbers. FAX quotations will be accepted only if dated and signed by an authorized company representative.
