Description

The Department of Veterans Affairs VISN 13 intends to issue a solicitation for Prescription Eyeglasses under Request for Quotation (RFQ) 618-12-01 for eligible veterans located in the jurisdiction of VISN 13 which includes the states of North Dakota, South Dakota, Minnesota, and selected counties in the states of Wisconsin, Iowa, Nebraska and Wyoming. Delivery must be in five days or less. The successful contractor must have fax capability. Period of performance is March 1, 2001 or date of award through February 28, 2002 with two (2) one-year renewal options, subject to the availability of funds. This procurement is a small business set-aside based on NAICS 421460 Ophthalmic Goods, size standard is 500 employees. This procurement is conducted in accordance with Federal Acquisition Regulation (FAR) Subpart 13.5 test program; simplified acquisition procedures apply. The solicitation adheres to the format defined in FAR Part 12, Acquisition of Commercial Items, Part 13, Simplified Acquisition Procedures and Part 15, Contracting by Negotiation. Solicitation to issue approximately January 4, 2001 with bid opening approximately January 29, 2001. Faxed proposals will NOT be accepted. See Note 1.
