Description
The VA North Texas Health Care System (VANTHCS) proposes to award a sole source contract to The John Peter Smith Health Network (JPSHN), a non-profit county medical facility operating in Tarrant County of Texas. The contract shall provide for dental care services for homeless veterans in the city of Fort Worth, Texas located within Tarrant County. These services are required as an intgral part of the Homeless Veterans Dental Program funded through a grant from the Department of Veterans Affairs Central Office. Under the program the contractor will be required to furnish said services one day per week for a period of one year to begin on January 16, 2001, or date of award, and terminate at midnight on December 31, 2001. Specifically JPSHN will provide the necessary personnel, equipment, and supplies to conduct basic dental exams and treatment. Provided services will be conducted at a location provided by the Salvation Army in conjunction with the VA North Texas Health Care System Homeless Dental program and the John Peter Smith Health Network. In general Terms, it shall be the responsibility of the Contractor to: 1. See eligible patients (homeless veterans in the Fort Worth geographical area, 2. Document veteran status by either: (i) Paperwork verifying enrollment in a VA-sponsored rehabilitation program or, (ii) Interview with the patient to determine veteran status, followed up by referral to Day Resource Center for enrollment in VA system, 3. Administer pre and post treatment surveys to measure quality improvement, 4. Invoice the VA on a monthly basis for a flat fixed fee as specified in the Schedule of Supplies/Services. Provider Qualifications: 1. Personnel assigned by the Contractor to perform professional services covered by the contract shall be licensed in a State, Territory, or Commonwealth of the United States of America or the District of Columbia. The qualifications of such personnel shall also be subject to review by the VAHVDP Coordinator, 2. Other necessary personnel, e.g. technicians, necessary for the provision of services shall be qualified at levels consistent with those of the State of Texas dental care industry. Personnel Policy: Consistent with it's internal policies, the Contractor shall be responsible for providing workers compensation; professional liability insurance; health examinations; income tax withholding; and social security payments. The parties agree that the Contractor's personnel shall not be considered as a VA employee. Work Hours: Services shall be provided on Friday of each week during the Contractor's normal clinic hours. Exception shall be those Fridays falling on a federal holiday. The 10 holidays observed by the federal government are: New Years Day; Washington's Birthday; Martin Luther King's Birthday; Memorial Day; Independence Day; Labor Day; Columbus Day; Veteran's Day; Thanksgiving; and Christmas. For contract purposes, any other day specifically declared by the President of the United States of America shall also be considered as a federal holiday. A solicitation will not be issued, however any interested person or firm qualifying and desiring consideration are requested to fully identify their interest and capabilities to the contracting officer's attention prior to 2:30 PM local on January 10, 2001.
