Description
The Department of Veteran Affairs Medical Center, San Francisco, CA intends to solicit and negotitate with only one (1) source under authority of FAR 6.302 for the Crti-Line III monitor and disposable blood chambers under the lease to purchase basis. The period of performance is one base year plus one option, with option to purchase the system at the end of the second year. The VA intends to purchase 10 Crit-Line III monitors which monitor Hematocrits/oxygen saturation monitors and approximately 5,184 disposable blood chambers per year. The Crit-Line III Instrument #17-400 consists of the following: Portable instrument that monitors absolute hematocrit (Hct), percent blood volume change, oxygen saturation, access blood flow and access recirculation. The auto scaling planar screen displays simultaneous graphic output of percent blood volume change vs. time as well as alphanumeric output of Hct. and oxygen saturation. Internal, non-volatile memory. Parallel DB25 and serial RJ45 data ports. UL 120 Vac/60 Hz-DC power supply. Patented. Disposable Blood Chamber #22-310 consits of the following: Individually packaged and pre-sterilized disposable blood chambers ready for use with the Crit-Line instrument. Provides a twist-lock attachment between arterial bloodline and dialyzer. Single use only disposable. Patented. Warranty, training, to be included. This announcement constitutes the only solicitation. A WRITTEN SOLICITATION WILL NOT BE ISSUED. This is a notice of intent to issue a sole source contract to In-Line Diagnostics Corp., Kaysville, Utah for the supplies sited above. Parties that feel that they can meet or exceed the above listed specifications may submit a proposal based on the information listed above, to the Department of Veteran Affairs Medical Center, 4150 Clement St., San Francisco, CA 94121, Attn: M.
