Description
Provide healthcare resources in accordance with Public Law 104-262 and 38 U.S.C. 8153. Contractor agrees to provide a Certified Registered Nurse Anesthetist (CRNA), as a qualified registered health care professional to and at the VA Roseburg Healthcare System. CRNA must be able to start on January 29,2001. If candidate is not available to start on January 29, 2001 offeror will be determined non-responsive, if after award it is determined candidate cannot start on January 29, 2001 contract shall be terminated for cause and award shall be made to next lowest offer. Price is an all-inclusive rate of per diem, travel, and price/cost. One CRNA 11 Work Days Monday Friday 7:30 am 4:00 p.m. January 29, 2001 February 12, 2001 On call Monday Sunday 4:00 p.m. 7:30 a.m.13 Call Days. January 30,2001 February 12, 2001 (Call services commence at 4:00 p.m. unless working overtime) Call back is estimated at 12 Hours (Call back rate shall be applied if CRNA is pager to return to the VA to perform duties) Overtime is estimated at six (6) hours. This shall apply only when the CRNA is not in a "on call" status or has not been "called back". This contract may require over timework. The government estimates 2 hours of over time per week. Over time must be approved by COTR or Contracting Officer prior to being taken. If over time is not prior approved it will be Denied upon invoicing. The CRNA must: a. Have an unrestricted license in a US state, territory or US commonwealth; b. Speak English proficiently; c. Have a minimum of five (5) years of experience; d. Be a US citizen; e. Have a professional degree and education applicable to medical staff members, e.g., CRNA; f. Provide evidence of current professional liability insurance; g. Evidence of current privileges (if applicable); h. Names of most current employer(s) and two peer reference contacts; i. Current copies of BCLS and ACLS certifications; j. Provide copy of current CRNA license; k. Be a team player; l. Must be computer literate, as VA Roseburg Healthcare System utilizes a computerized patient medical record; m. Nationally certified required and Oregon licensed preferred; n. Proficient in: 1. Administration and management of general anesthesia to include but not limited to: general anesthesia, regional spinal, epidural and nerve blocks; 2. Function as an independent practitioner; 3. Placement of central lines, arterial catheters and pressure monitoring; 4. Manage difficult airways; 5. Pre-Anesthesia Assessment; 6. Ventilator management. o. Must be flexible in working with a variety of surgical entities as well as performing procedures outside the OR, i.e. central line placement. Contractor must provide the following documentation along with the candidate's information: 1. Evidence of current TB surveillance status. 2. Evidence of current Hepatitis B vaccination and titer. 3. If the contract physician is required to work in an environment that requires a respirator, the contractor will either provide their physician/healthcare provider with a properly fitted respirator or the contract physician will be required to go through VA Roseburg's Respiratory Fit Program prior to beginning their assignment. Provider assigned by the contractor to perform the services stated in this contract will be credentialed by and receive privileges from the VA. Types of surgery performed at the VA Roseburg Healthcare System include but are not limited to: general, urology, thoracic, ophthalmology, ENT, podiatry and endoscopy (for IV sedation and monitoring). CRNA will be on call weekdays from 1600 (unless working overtime) to 0730 the following weekday. Weekends they shall be on call from 1600 (unless working overtime) Friday until 0730 Monday. CRNA must report to the VA Roseburg Healthcare System (VA) within 30 minutes upon being paged. When paged the CRNA shall respond to the pager via phone then report to VA within 30 minutes. While on call the CRNA must be able to respond within 30 minutes. In the event that the CRNA is unable to respond to the page the VA retains the right to transfer the patient for care in the community at the Contractors expense. Contractor shall dictate a concise and comprehensive report using VA telephone dictation processes. Solicitation packages will be available after 12/27/00.
