Description

Provide Registered Dental Hygiene services at the Coast Guard clinics located at Seattle WA. to beneficiaries referred for treatment by a Coast Guard Dental Officer. This will be a personal service contract. This requirement will be accomplished utilizing Far part 13, Simplified Acquisition Procedures . The contractor will work weekdays during hours established by the Clinic Supervisor, normally not to exceed 8 hours a day, 16 hours a week for a total of 832 hours per year. Period of performance will be 29 January 2001 through 31 December 2001. The Request for Proposals will be issued on or about 22 November 2000 and close on or about 31 Dec 2000. This procurement is a Small Business Set-Aside. The NAIC Code for this acquisition is 621399, the SIC code is 8049 and the small business size standard is $5.0 million average annual receipts for the proceeding four fiscal years. All responsible sources may submit a bid, which shall be considered by the agency. All requests for solicitation shall be in writing.
