Description

STATEMENT OF WORK The Contractor shall provide professional medical and related services as specified herein. All treatments shall be performed at the Contractor's place of business in accordance with all terms, conditions, provisions, specifications, and schedules herein. The Contractor shall provide radiation therapy consultation, treatment, and follow-up care for beneficiaries of the Department of Veterans Affairs as authorized by the VA Western New York Healthcare System, Buffalo, NY. The Contractor must have sufficient resources to support the increased workload of the VA population and provide quality care of veteran patients including adequate qualified/certified personnel, seeing and treating patients in a timely manner and adequacy of planning and treatment equipment. Radiation therapy consultation shall include examination of the patient and the patient's medical record or copies thereof. In performing radiation therapy treatment, the contractor shall provide complete palliative or curative treatment, in various modalities, as necessary. All radiation therapy services are to be provided under the direction of a board certified radiation oncologist. Requests may be faxed, e-mailed or submitted on letterhead via US postal services. No telephone requests will be accepted.
