- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-20. Request for Quotation (RFQ) 558-01-50 should be referenced on all offers. The North American Industrial Classification System (NAICS) code for this procurement is 621111 and the small business size standard is $5 million average annual receipts for the past three years. Offerors are encouraged to submit quotes based on an estimated 520 hours. Proposed rates should include all costs associated with providing the following LOCUM TENENS, Radiology Staffing Services. The successful offeror shall provide (1) Diagnostic Radiologist and/or (2) Radiology Technologist. Individuals proposed to work under this contract shall be board certified in diagnostic radiology by the American College of Radiology and American Registry of Radiologic Technologist. Desired qualifications would include experience in interpretation of body section radiographs (tomograms, CT and ultrasound exams), GI/GU exams, portable exams and intra-operative studies, portable radiographic and fluoroscopic units, and general and high quality diagnostic radiographs. Desired additional qualifications would be mammography as defined by the ACR and FDA and MRI skills. Copies of Board Certifications, current licenses, Curriculum Vitaes, documentation of education, training and experience are to be forwarded with proposals. The contracting agency is required to provide documentation of malpractice insurance for all service providers,prior to contract commencement for Radiologist and after contract commencement for technicians. In accordance with the DVAMC Compliance Plan, the contractor/agency is to ensure that individuals do not appear on the current Office of Inspector General's Exclusion List from participation in Medicare, MEdicaid and other Federal Health Care Programs(http:epls.arnet,gov/epl/owa/cpls). Initial contract period will be 30 days upon credentialing and approval with option to extend 90 days. Total contract period not to exceed 120 days. The agency will notify the DVAMC at the time of proposal of any special accommodations that might be required for potential service providers. The daily work schedule would be full time, 8:00 a.m. to 5:00 p.m., Monday through Friday. Overtime work will be authorized in advance on a case by case basis. Radiologist call rotations are every fifth weekend, 5:00 p.m. until 8:00 a.m. Monday with mandatory on-site service 8:00 a.m. -- 11 a.m.. Payment for holidays and any leave, including sick leave, holiday or vacation time shall be the responsibility of the contractor. All services shall be performed "on-site" at the VA Medical Center, Durham NC. The contract Radiologist shall be required to interpret radiologic examinations and radiographs, provide evaluation of patients and consultations as required, perform verbal dictation of interpretations using RTAS digital dictation system,review and correction of typed reports with electronic signature, hang radiographs and occasional supervision and education of residents when required. Service providers may be subject to random drug screenings as determined by Human Resource Management Service upon commencement of contract. Failure to pass a required drug screening, or evidence discovered after start date that would cause revocation of privileges, will cause the immediate removal of the contract personnel,cessation of billable hours and a requirement by the contracting agency to provide another qualified service provider. Failure to provide an approved individual for two consecutive weeks after contract commencement date will be grounds for default. The provision at 52.212-1, Instructions to Offerors Commercial (OCT 2000), applies to the acquistion. The provision at 52.212-2, Evaluation-Commercial Items (JAN 1999), applies to this procurement. Offerors are requred to submit required verifications and certification documents with proposals. Evaluation criteria included below are in descending order of importance, (1) Past Performance, (2) Competency and (3) price. Past Performance and Competency are significantly more important than Price. Offerors are required to include a completed copy of the provision at 52.212-3 Offeror Representations and Certification-Commercial Items ALT III (OCT 2000) with its offer. The clause at 52.212-4 Contract Terms and Conditions-Commercial Items (AUG 2000), applies to this acquisition with the following clauses added as addenda to the clause:52.204-4 Printing/Coping Double Sided on Recycled Paper (JUN 1996), 52.204-6 Data Universal Numbering System (DUNS) Number (APR 1996), 52.224-1 Privacy Act Notification (APR 1984), 52.224-2 Privacy Act (APR 1984), 52.217-8 Option to Extend Services (APR 1989), 852.237-7 Indemnification and Medical Liability Insurance (OCT 1996), 852.270-1 Representatives of Contracting Officers (APR 1984), 852.219-70 Veteran-Owned Small Business (DEC 1990).
