Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation is issued as a Request for Quote under the number F73MED030000300. This solicitation is reserved as a small business set-aside. The associated Naics code is 511210, and the small business size standard is $18M. Quotes are requested on a Brand Name or Equal for the following: T-System or equal, an emergency room charting system to document patients total past health history and current diagnostic symptoms. Software must be template driven and templates shall cover at least 95% of patients treated in the emergency department. Templates shall ensure a complete evaluation of each patient through the use of prompts, allow for complete documentation by the physician by reducing the amount of handwriting required of the physician and be accepted for reimbursement purposes by federal and state agencies, Health Maintenance Organizations, and insurance companies. All upgrades to the software and the templates must be provided by the supplier as soon as they are available. Software shall comply with HCFA documentation rule. Services shall include assessing existing system of patient chart flow and provide advice about locating the special shelving and access reproduction capabilities. Offeror is also to provide a method for in-house reproduction capabilities of emergency room charts and a moveable shelving unit to store charts. When updates and upgrades of charts become available, they will be provided to emergency room personnel from the time of award to 30 Sep 01. Offerors are required to submit samples of all charts offered in this requirement and the technical literature of shelving storage and method of reproduction capabilities. Contractor will provide orientation training of emergency room charting system usage to hospital personnel, and continued training for in-coming personnel. Contractor also to provide problem-solving (troubleshooting) solutions during the phase in period. Offerors are required to submit a completed copy of the provision FAR 52.212-3, Offeror Representations and Certification _ Commercial Items, with their offer (Atch 1). To assure timely and equitable evaluation of offers, each offeror must complete and submit a Technical Proposal, Price Proposal and Past Performance Information. Award of this acquisition will be based on Best Value to the Government using technically acceptable, price, and past performance as the evaluation factors. The Government reserves the right not to award to the lowest quote. Price will be evaluated by the total of all items added together. Technical Proposal shall consist of manufacturers' technical specifications, literature, and brochures of items offered as "or equal". Past Performance will be evaluated on basis approximately equal to, price considerations. Offerors will provide a list of three (3) of the most relevant services performed for either Federal or commercial customers within the last three years. Relevant services, are services, (requirements) that provided emergency room charting system software for facilities similar in size to Scott Air Force Base that treats an average of 2000 patients on a monthly basis. Provide the following information for each reference if applicable. Company name, address, point of contact, exact service performed, dollar amount, period of performance, phone number, fax number and E-mail address.
