Description

The Sierra Pacific Network Consolidated Contracting Activity (CCA) is issuing a Commercial Item Request for Proposals (RFP) in accordance with FAR 12.203 and FAR 15 to provide Medical Transcription Services for the Department of Veterans Affairs Sierra Nevada Healthcare System, 1000 Locust St., Reno NV 89502, a Department of Veterans Affairs facility within the Veterans Integrated Service Network (VISN 21). The contractor shall supply all labor, equipment, transportation and supervision necessary to transcribe discharge summaries, operation reports, procedure reports, stat hospital summaries, compensation and pension (C&P), progress notes, and other work types as required. Contract period is March 1, 2001, or date of award, through February 28, 2002 with four one-year renewal options. The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors Commercial Items; 52.212-2 Evaluation Commercial Items; 52.212-3 Offeror Representations and Certifications -- Commercial Items; 52.212-4 Contract Terms and Conditions Commercial Items; and 52.212-5 Contract terms and Conditions to Implement Statutes or Executive Orders Commercial Items.
