Description
This is a COMBINED SYNOPSIS SOLICITATION for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. A PROPOSAL IS BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. (ii) Solicitation No. 01WRSS0002 is issued as a Request for Proposal (RFP). (iii) This solicitation documents and incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 90-21. (iv) The North American Industry Classification System (NAICS) code is 811219, with a small business size standard of $5.0 Million. The US Geological Survey, Menlo Park, CA has a requirement for service maintenance of a WR, JEOL 8900 Microprobe. This procurement is being procured on a sole-source basis from JEOL USA, INC., Peabody, MA. The contractor shall provide service maintenance for WR, JEOL 8900 Microprobe, Serial #120004-08 located at the US Geological Survey, Menlo Park, CA for the a base period, plus four option years. The unit price for each period is on a per month basis. Remedial maintenance will be paid on a labor and materials basis. Therefore, the Contractor shall provide an hourly rate for the base and four option years for the services of a technician. A parts list shall also be provided for the base and each option year. The base and each option year will have a Not-To-Exceed amount for this Contract Line Item. The contractor shall provide all parts, materials, labor and transportation required to perform preventative and remedial maintenance on the government-owned system. The system is located in Building 11, 345 Middlefield Road, Menlo Park, CA. The Contractor shall provide all parts, materials, labor and transportation required to perform preventive and remedial maintenance on the government-owned system. The system is located at Building 11, 345 Middlefield Road, Menlo Park, CA. All parts, materials, and components, except those expendable items noted below shall be replaced when necessary during preventive and remedial maintenance calls at no additional cost to the Government. Replacement parts shall become the property of the Contractor. Those expendable items for which the Contractor shall not be responsible are limited to filaments, apertures, specimen exchange tools and specimen holders. The work to be performed shall be in accordance with the original equipment manufacturer's specifications and recommendations. The Contractor shall maintain an inventory of spare and repair parts and such tools and instruments as necessary to properly and efficiently maintain the equipment for the life of the contract. All service under this contract are to be performed by competent personnel, experienced and certified to provide required services in accordance with the best commercial practices, without unnecessary delays or interference with Government functions. Preventive Maintenance: Preventive maintenance shall include, but not be limited to, cleaning, adjusting, lubricating, inspecting, and testing procedures to keep the equipment in good operating condition, preclude equipment failures to the greatest extent possible, and extend useful equipment life. It includes running of all diagnostics, and repair and replacement of all defective parts. Preventive maintenance shall be performed in accordance with the manufacturer's recommended maintenance schedule during the performance period of this contract. The Contractor shall furnish a legible service report for each preventive maintenance call to the Contracting Officer's Representative. Preventive maintenance scheduled within seven (7) calendar days of remedial maintenance call may be performed during that remedial maintenance call period, however, at least five months have elapsed since the last preventive call. Preventive maintenance performed under these conditions, if outside the schedule set forth above, Preventive maintenance (PM) shall be performed during the Principal Period of Maintenance (PPM) for the Option years. PM shall be performed twice a year at 6-month interval periods. Remedial Maintenance: Remedial maintenance shall include, but not be limited to, the diagnosis and correction of equipment malfunctions and failures. It shall be performed on an on-call, as needed basis throughout the performance period of this contract. Remedial maintenance shall include the provisions of all transportation, labor, and parts required for return of malfunctioning system to normal operating condition. Remedial maintenance shall be performed during the Principal Period Maintenance on an on-call, as needed basis. Overtime and holiday premiums are not authorized. The Contractor shall furnish a legible service report, capable of being copied, for each remedial maintenance call. The service report shall be delivered to the Contracting Officer's Representative. Remedial maintenance shall commence within three days after telegraphic or telephonic notification of a malfunction by the Contracting Officer's Representative or his designee. Remedial maintenance shall include the provisions of all labor and parts required for return of the system to normal operating condition within forty-eight (48) consecutive hours after commencement of the repair for the system under this maintenance contract. Consecutive hours are defined as including all clock hours, both inside and outside the PPM. Clauses/provisions: FAR Clause 52.212-1 "Instructions to Offerors-Commercial," is hereby included by reference. The following are addenda to FAR Clause 52.212-1: Paragraph C, Period for acceptance of offers; the Offeror agrees to hold prices on their proposal form for 60 calendar days from the date specified for receipt of offers. Offerors are advised that award may be made without discussions. Award will be made to the responsible offeror whose proposal is responsive to the terms of the RFP and is most advantageous to the Government. The offeror is required to provide past performance information for same/similar services, to include two most recent contract numbers, points of contact/current telephone numbers, type service provided. FAR provision 52.212-2, Evaluation-Commercial Items applies to this solicitation and the following evaluation criteria (arranged in their order of importance) shall be used to evaluate offers: Qualifications (experience of offeror), Past Performance, and Price. Offeror will be evaluated based on the quality and applicability of their experience in work similar to and comparable to the scope of these specifications. Provide information on the offeror's experience maintaining JEOL, 8900 Microprobe over the past 5-year period. Attach a list of at least three satisfied customers with name, address, telephone number and responsible person to contact if the Government wishes to check these references. FAR provision 52.212-3 Offeror Representations and Certifications-Commercial Items shall be completed and submitted with the proposal. FAR Clause 52.212-4, Contract Terms and Conditions-Commercial Items, hereby applies to this solicitation. FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items applies to this acquisition with the following addenda: Paragraph (b) the following FAR clauses are hereby incorporated by reference: 52.203-6, 52.222-26, 52.222-35, 52.222-36, 52.222-37. Paragraph (c) the following clauses are incorporated by reference: 52.222-41 and 52.222-42.
