Description
BWXT of Ohio, Inc. (BWXTO), Prime Contractor at the U.S. Department of Energy's (DOE's) Miamisburg Environmental Management Project (MEMP), Miamisburg, Ohio, seeks Qualified Source(s) to provide for the decontamination and demolition (D&D) of the Building 38 complex. Only pre-qualified offerors will be afforded further consideration. The scope of work for the subcontract expected to be awarded will include, but not be limited to, the environmental remediation of the Building 38 Complex including decontamination and demolition (D&D) of Building 38, associated structures, vent stack and soils contaminated with both radiological (Plutonium and transuranics) and hazardous constituents (asbestos, lead, limited areas contaminated with beryllium). Building 38 is a two level, 44,000 sq ft reinforced concrete structure and associated 200 ft tall masonry vent stack. Within the building are various contaminated and non-contaminated equipment items. Contaminated items include approximately 29 glove boxes approximately seventy feet long (both stainless steel and fiberglass) and 6 fume hoods. Other equipment may be contaminated. All waste generated will be packaged in waste containers provided by BWXTO. Used waste containers will be returned to BWXTO for final disposition. The ultimate Subcontractor will not handle any D & D project waste off site. The entire D&D activity addressed herein will be conducted as a CERCLA Removal Action. Offerors requesting to become pre-qualified shall submit information to address all the following: 1) Project Experience. a) List two similar projects with magnitude of value at least $2,000,000 each that the Offeror has performed during the last five years. The listed projects must be similar in scope and involve work of the same complexity as the work that will be required to remediate and D&D plutonium contaminated buildings, vent stack and soils associated with the Building 38 complex. The Offeror must have performed these projects as the prime contractor and must have self-performed over 50% of the work. b) Provide the name, title, fax number, and telephone number of two client references for each project. These references must be willing and able to address Offeror's safety and environmental record, schedule performance, and cost control. 2) Corporate Training Program. Provide documentation that substantiates the training programs that will be used to ensure all individuals the Offeror may utilize for performance of the scope of work described above, will be trained to perform the required work. 3) Environmental Safety & Health (ES&H) Program. Provide documentation that substantiates Offeror's ES&H program includes policies and procedures that address, at a minimum the following: a) Elimination of accidents, injuries/illnesses, and damage to property and equipment; b) Hazard identification and mitigation; c) Audits performed to assess the effectiveness of the program; d) Hazard Communication Program including maintenance of Material Safety Data Sheets (MSDS). e) Employees' right to voice safety and health concerns without fear of reprisal, should they feel their safety or health is being compromised. f) Define responsibilities and provide qualifications of site safety personnel. 4) Safety and Compliance. a) The Offeror must demonstrate familiarity with an Integrated Safety Management program that conforms to the Department of Energy Acquisition Regulation 970.5204-2. b) Offeror must demonstrate a record of safe performance. Provide documentation of current interstate workers' compensation insurance experience modification rate (EMR) of less than or equal to 1.00; or show a downward trend for the past three years. EMR for each of the past three years cannot exceed 1.15. An Offeror that is too small, too new, or self-insured, to provide EMR records may submit its total OSHA recordable rate. Acceptable rates will be less than or equal to 8.8 cases per 200,000 man-hours worked. c) Offeror must provide copies of its OSHA 200 log, "Summary of Occupational Injuries and Illnesses" for the most recent three full years. Offeror must list the number of OSHA citations received in the last five years. List the type, the standard cited, and the location where the citation was issued. If no OSHA citations have been received, provide a signed original certification that Offeror has not received any citations within the last five years. d) Copy of any fines and findings issued regarding Offeror's compliance with the Price-Anderson Amendments Act (PAAA). If no PAAA fines or findings have been received, provide a signed original certification to this effect. f) Copy of any fines and findings issued by the Nuclear Regulatory Commission (NRC) regarding Offeror's performance. If no NRC fines or findings have been received, provide a signed original certification to this effect. 5) Plutonium Glove Box removals. Provide description of experience with handling and removal of glove boxes, contaminated with plutonium to transuranic waste (TRU) levels. 6) Nuclear Facility Management Experience. Provide information on Offeror's experience with management and maintenance of DOE Nuclear Hazard Category 3 or greater facilities. Management experience will include Offeror's capabilities with DOE Authorization Basis activities, Unreviewed Safety Question (USQ) determinations and environmental release control. Maintenance experience documentation must include offeror's capabilities with fire detection and suppression systems, HEPA filtered HVAC systems and contaminated liquid sump, drain and tank systems. 7) Financial Capability; a) Provide a letter from the Offeror's bonding company setting out the bonding limits provided to the Offeror; b) Provide copies of the Offeror's two most recent annual reports. 8) Quality Program. a) Provide a copy of the Offeror's Quality Assurance Program (QAP) and b) a certification that the Offeror's QAP meets the requirements or is familiar with DOE Order 414.1 and 10 CFR 830.120. 9) Labor Relations. Provide evidence that the Offeror has labor relations experience in union environment. 10) Support for Small and Small Disadvantaged Businesses. Provide documentation to substantiate Offeror's commitment to support of Small Business and Small Disadvantaged Businesses. (Note, Small Business requirements for the project will be identified in the solicitation documents. Formal plans will be required and submitted with the Offeror's proposal). 11) References. Provide a list of all projects with a price of $5,000,000 or greater that the Offeror has performed during the last two years. For each project listed, provide a) the name, fax number, and telephone number of the client point of contact, b) dollar value, c) description of scope, d) contract type, dates of performance and name of client for whom the work was performed. 12) If known, Offeror shall provide name(s), address(es), and point(s) of contact, and area(s) of expertise for potential teaming partners and/or subcontractors that may be employed to augment the Offeror's effort on the project. A qualified Offeror is one whose response to this announcement demonstrates that it possesses the requisite knowledge and experience necessary to successfully perform the environmental remediation and D&D of buildings, vent stacks, and soils contaminated with both radiological and hazardous constituents. Pre-qualification of potential offerors is at the sole discretion of BWXTO. The omission or failure to correctly and satisfactorily respond or certify to any pre-qualification criterion, as required in this announcement, may result in the Offeror's exclusion from further consideration by BWXTO. No other information, except as specified herein (i.e.: cover letter addressing pre-qualification responses/certifications, e-mail addresses, and attachments) shall be included in the Offeror's submittal package. Submittal information packages shall be limited to 50 pages (not including Quality Assurance Program). Request for proposal documents will be only issued to and proposals accepted from qualified Offerors. The estimated magnitude of this project is between $5 million and $10 million (U.S. Dollars). Expressions of interest, with the required submittal information, must be submitted in writing to BWXT of Ohio, Inc., Attention: Ms. Deborah Bloor, (M/S OSE-214G), 1 Mound Road, Miamisburg, Ohio 45343 by 5:00 PM Monday, February 26, 2001. The anticipated issuance date for the Request for Proposal is May 2001. Telephone requests will not be accepted.
