Description

The Department of Veterans Affairs, VA Roseburg Healthcare System, Roseburg, Oregon, requires a contractor to provide audiology services which will include the following: 1) audiogram; 2) hearing aid evaluation/fitting; 3) binaural hearing aid fitting; 4) monaural hearing aid fitting; and 5) earmold. The contractor shall perform services at contractors site, which shall include all labor and materials to perform the audiology services required in the contract. This will be a multiple-award contract. One contract award per county, within the following areas: Coos, Curry, Douglas, Lane, Jackson, Josephine, and Klamath. This contract requirement is a base year, plus four-(4) one-year options contract pending availability of funds. Contract performance will be required from April 1, 2001 through March 31, 2002 as the base year. This solicitation is set-aside for 100% small business. NAICS Code 621340/$5 million applies.
