MidSouth Healthcare Network (VISN 9) Acquisition Service Center is issuing a commercial item Request for Quotation solicitation in accordance with the format in Subpart 12.6 and 13.5 (test program), as supplemented with additional information included in this notice. Award will be made using a combination of FAR Parts 12, 13, & 15. Requirements are for the contractor to access the Lexington, KY VAMC, Memphis, TN VAMC, and the VA Tennessee Valley Healthcare System, consisting of the Nashville Campus and Alvin C. York Campus, including the VA Outpatient Clinics in Knoxville, TN and Chattanooga, TN, dictation systems. Information will then be transcribed and downloaded into the Medical Centers computer database. Any estimated long distance phone charges are to be built into the contract price and will be the responsibility of the contractor. Various turn-around-times will apply. The VA Medical Center, Huntington, WV will utilize the contractors location central dictation system, which should be accessible via toll free telephone service from anywhere in the continental United States. The information must be downloaded in the Huntington VA computer database. Contract period is April 16, 2001, or date of award through April 15, 2002, with two one-year renewal option periods. The place of performance is unknown. Wage rates have been requested for Nashville, Murfreesboro, and Memphis Tennessee, along with Lexington, Kentucky and Huntington, West Virginia. Contracting Officer will request Wage Rates for additional places of performance if asked to do so in writing by February 15, 2001 via fax to 615-867-5429. Quotes are due back by February 26, 2001, at 2:00pm, local time. The NAICS code is 561410 with a size standard of $5.0 million.
