Description

DESC: Market Survey/Request for Information. The Centers for Disease Control and Prevention and the Agency for Toxic Substances and Disease Registry in Atlanta, GA. will be requiring counseling and referral services for Child and Elder Care, Health and Emotional Wellness, Family Services, Educational Information and Financial Planning for employees nationwide including Alaska, Hawaii and Puerto Rico. Contractor shall provide consultation to approximately a 10% _ 15% utilization base of approximately 8600 employees annually to assist in identifying options available for caring for family, dependent children and older adults, evaluating these options and referral of appropriate services. In addition, the contractor shall provide informational and/or educational materials appropriate for 8600 employees during the base period and an additional 500 informational packets for each option year and provide 24 hour telephone access to counselors. The Government is seeking innovative, efficient, effective and practical approaches to work life resource and referral services and to cost effectiveness. This information may provide direction for formulation of possible contractual vehicle. The Government is further seeking information on Standard Commercial Industry Practices, Contractual Relationships, Labor Mix, Pricing, Methodologies and Approaches, Quality Control and Assurance Practices. Information submitted shall include: (1) telephone and fax number of point of contact (2) Operational NAICS Code(s) and (3) Type of business (i.e.: small business, small disadvantaged 8(a) certified, Woman-owned small business, small disadvantaged business (not 8(a) certified). This market survey/request for information is for planning purposes only, and shall not be construed as a Request for Proposal (RFP) or as an obligation on the part of the Government to acquire any follow-on acquisition. The Government does not intend to award a contract on the basis of this market survey/request for information or otherwise pay for the information solicited. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of the submission of responses to this market survey/request for information. The Government reserves the right to reject, in whole or in part any industry input as a result of this market survey/request for information. This market survey/request for information does not constitute the solicitation of any proposals and authority to enter into negotiations to award a contract. Although the Government would prefer to review non-proprietary information, proprietary information will be accepted.
