Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. Solicitation number RFQ-797-FDF4-01-0014 is issued as a Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-22. The NAICS code is 339112 and the small business size standard is 500 employees. This action is set aside for small business. This solicitation is for the 60th Medical Group, Travis AFB, CA. This RFQ is for brand name only. The contractor shall provide the following: Quantity of 11 each Part No. 450:00-9300N-T Xenon 300 watt L/S w/Turret; 11 each Part No. 450:AX-1370-BIF-EL Ultralite 2 w/extended linkage, 11 each Part No. 450:00-1400 Floorstand w/360d rty sw bs and 2 each service manuals and 2 each operator's manual. The Government requests a trade-in allowance of 11 each light sources. Delivery is FOB destination to Travis AFB, CA 94535-1800 no later than March 28, 2001. The following FAR clauses apply to this acquisition. FAR clause 52.212-1, Instructions to Offerors -- Commercial Items; FAR Clause 52.212-4, Contract Terms and Conditions -- Commercial Items; FAR Clause 52.212-3, Offeror Representations and Certification -- Commercial Items; and FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certification -- Commercial Items, with the offer. The Government anticipates award of a purchase order resulting from this solicitation to the responsive/responsible offeror whose offer conforms to the solicitation and is most advantageous to the Government.
