Description

The Contractor shall provide all necessary personnel, supplies, and equipment to construct/provide new prosthetics, repair prosthetics, and new orthotics, repair orthotics, post operative temporary prosthetics, prefabricate orthopedic appliances and provide consultation regarding fit and suitability of orthopedic appliances. The Contractor shall also provide brace & amputee, and/or spinal cord injury clinics at the following Navajo Area Indian Health Service hospitals/health center: Chinle Comprehensive Health Care Facility, Chinle, AZ; Fort Defiance Indian Hospital, Fort Defiance, AZ; Tuba City Indian Medical Center, Tuba City, AZ; Dilkon, AZ; Winslow Health Center, Winslow, AZ ; Crownpoint Health Care Facility, Crownpoint, NM; Gallup Indian Medical Center, Gallup, NM; and Northern Navajo Medical Center, Shiprock, NM. Contract to be effective for a 12-month period with two (2) one (1) year option periods. Interested bidders should submit their request for IFB: M01-0019/MD. Bid Opening Date: March 30, 2001. This solicitation is a 100% set-aside for small business.
