- This is a combined synopsis/solicitation for a supply contract in accordance with the format in Subpart 12.6, as supplemented with the additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITTION; PROPOSALS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation number is GS442-01 and this is a Request for Proposal. The Standard classification code SIC Code is 2653- Boxes. Proposals are due in this office Wednesday, April 11, 2001 at 1:00 PM ET not 2:00pm ET as printed on the SF1449 form in the solicitation. This contract will be a requirements type contract for Boxes at the UNICOR, Federal Prison Industries, Inc. UNICOR LaTuna factory located in Anthony Texas for a period of performance of one base year and one option year (if exercised). The items to be procured are a variety of packaging boxes. The boxes will be manufactured in accordance with Federal Specification PPP-B-566E dated 08/02/74 and amendment 3 dated 11/28/80. Prices will be FOB Destination to the UNICOR La Tuna Factory in Anthony, Texas. Deliveries will be between the hours of 7:30 am 11:00 am and 12:00noon-1: 30 pm excluding weekends, Federal Holidays, and institution emergencies. Bid Samples will be provided by all contractors who have not previously provided these items to UNICOR LaTuna. Two packs for each item bid must be provided prior to bid opening. Samples will be tested/checked for conformance. Boxes shall be free from dirt, tears, scabs, blows, and as applicable, unsecured cover paper and dimensions. Samples are due by the solicitation closing date and time. Contractors will send two packs for each item to Ms. Elke Mosholder, Quality Assurance Manager, UNICOR LaTuna, 8500 Doniphan Road, Anthony Texas 79821. The contractor will only deliver items place by individual delivery orders by authorized personnel at LaTuna, Texas, and/or Central Office in Washington, DC. With the exception of the Contracting Officer's Technical Representative. Variations in quantity per each delivery order are plus or minus 5 %. Items are due to UNICOR within 45 calendar days after receipt of order. Delivery order limitations are a minimum of 2000 each per line item and 5,000 each per delivery order. The maximum quantities are 25,000 per order. The maximum quantity per the life of the contract is 1,034,000 each. The Contractor is required to submit a Certificate of Conformance with each order. The proposal will be evaluated by the following Best Value criteria: Price, Past Performance, Quality and Delivery. This solicitation will result in a Multiple Award. The offeror is required to provide past performance information for the same or similar product from two of the most recent contracts, include the following information: Company name, contact person, telephone number, product/service provided, and dollar value of the contract. The Government reserves the right to make award on the initial offer received without discussions. Offerors shall submit signed and dated offers on Standard Form SF 1449 or Company letterhead stationary. Facsimile proposals are acceptable at 202/305-7353, to the attention of Alice Calabro, Contracting Officer. Offers must be received no later than Wednesday, April 11, 2001 at 1:00PM ET not 2:00PM ET as stated on the SF 1449 form in the solicitation.The proposal must include the company name, address, telephone number, technical description, of the items being offered in sufficient detail to evaluate in compliance with the requirements in the solicitation, prices and a completed copy of Representation and Certifications Commercial Items and acknowledge all amendments, if applicable. Offeror will hold prices firm for 60 calendar days from the closing date specified in the solicitation. If the offer is not submitted on a Standard Form SF 1449, submit the proposal on Company letterhead, with a statement specifying agreement with the terms and conditions of the solicitation. Offers that fail to furnish required representation and certifications or reject the terms and conditions of the solicitation may be excluded from consideration. All responsible/responsive contractors will be considered for award. A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer shall result in a binding contract without further action by either party. Before the offer specified expiration date, the Government may accept an offer or part of an offer after its receipt unless a written notice or withdrawal is received before award. The offeror must specify business size.
