- Description
- This is a combined Synopsis/Solicitation for commercial items prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in this notice. This procurement is being conducted under the Simplified Acquisition Procedures in accordance with FAR Part 13. This announcement constitutes the only solicitation; this is a total small business set aside, size standard 7 million; NAICS code for this requirement is 325412; quotes are being requested and a written solicitation will not be issued. This is a request for quotation (RFQ), solicitation number is RFQ 652-239-01. This announcement incorporates provisions and clauses in effect through Federal Acquisition Circular 97-23 and VAAR 97-8. Provide F18 Fluorodeoxyglucose (F18 FDG) to the Hunter Holmes McGuire VA Medical Center, 1201 Broad Rock Blvd., Richmond, VA 23249 for the period of 5/1/01 through 9/30/01 on an as needed basis with the provision for four (4) one-year option periods. Radiopharmaceutical must be calibrated and ready for use when received at Medical Center. Orders will be placed by Nuclear Medicine Imaging Staff as needed. FOB destination pricing is requested. Quantities are estimated. Base period 4/1/01- 9/30/01 -- estimated quantity 15 ea.; option years estimated quantity 30 ea..
