Description

This is a SOURCES SOUGHT notice. VAMC Alexandria LA plans to negotiate a sole source contract with Freedman Clinic of Internal Medicine for the services of medical doctors specializing in NEPHROLOGY to treat veteran referrals with all phases of End Stage Renal Disease. Services are required at the office(s) of the physician(s)and multiple dialysis centers within a 60-mile radius, including Alexandria, Pineville, Bunkie, and Leesville LA. The intent is to award a Firm-Fixed price Requirements Contract prepared in Commercial format. Note Number 22 applies. Estimated number of referrals is 10 per year. A base year and four optional one-year provisions are included. This is an unrestricted procurement. Physicians shall be licensed US citizens who are Board Certified or Board Eligible Nephrologists and meet all phases of medical center Credentialing and Privileging. Services shall be provided in accordance with physician's Current Procedural Terminology, latest edition, and reimbursed at rates not exceeding Medicare Allowables for this locale.
