Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotations are being requested and a written solicitation will not be issued. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with the commercial item procedures of FAR Part 12. The solicitation is being issued as a Request for Quotation (RFQ). The solicitation number is RFQ 652-249-01. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-23. The NAICS is 339112 and the small business size standard is 500 employees. The procurement is unrestricted. The Hunter Holmes McGuire VA Medical Center, Richmond, VA intends to solicit quotations and award a firm fixed price contract on a competitive basis to the offeror whose quotation meets the requirements of this solicitation. Period of contract will be established as one calendar year from date of award with one renewable option year. Quotations are due by close of business March 27, 2001, local time. All responses should be for specified manufacturer/brand name or equal. Quotations shall be submitted on an "all or none" basis. Partial quotations will be rejected. The Hunter Holmes McGuire VA Medical Center intends to enter into a cost- per-procedure arrangement with a contractor to provide an Alcon Phaco-Emulisfication System for use on site at the Medical Center. The system shall include the following: 1) Product No. STTL-C STTL Console Only, Qty 1; 2) Product No. Turbosonic 375 Enhanced Legacy 375 Turbo Phaco H/P, Qty 4; 3) Product No. 8065740738 Drive Mechanism, Steerable I/A, Qty 1; 4) Product No. 8065740739 Handpiece, Kelman, Steerable I/A, Qty 3; 5) Product No. 8065814101 Ultraflow I/A Handpiece Sets, Qty 2; 6) Product No. 8065803602 Sterilization Tray (Phaco Handpiece) Qty 4. The VA will pay the contractor a firm fixed price for each procedure performed utilizing the equipment. The VA will purchase from the contractor one Product No. 8065740827 Machine Pack for use during each procedure. VA estimates 15 procedures will be performed per month. Quotations must include the following: 1) descriptive literature and product number for equipment offered; 2) equipment use charge/cost per procedure; 3) cost and product number of each item VA will need to purchase for use with equipment; 4) evidence of past performance of contracts of similar type, scope size and complexity that are ongoing or completed within past three years, references with points of contacts for contracts must be identified. 5) Delivery time, F.O.B point, shipping weight and delivery costs. The following provisions apply to this solicitation: 52.212-1, Instructions to Offerors-Commercial Items; 52.212-4, Contract Terms and Conditions-Commercial Items and Addendum 52-217-9, Option to Extend the Term of the Contract; 52.232-18, Availability of Funds; 52.232-19, Availability of Funds for the Next Fiscal Year; 52.211-6, Brand Name or Equal; 852.211-77, Brand Name or Equal; 852.270-4, Commercial Advertising; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items including clauses incorporated by reference under Section b (11-16)(18)(21)(24). Award will be made to the offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered (capability of equipment to meet the governments need and past performance). The government intends to evaluate offers and award without discussions. However, the government reserves the right to conduct discussions later if determined necessary by the Contracting Officer. The information required in provisions 52.212-1, 52.212-2, a completed copy of provisions 52.212-3, Offeror Representations and Certifications-Commercial Items, and 852.219-70, Veteran Owned Small Business certification must be submitted with offer.
