Description
RFP 671-54-01, this is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in far subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; request for proposal (RFP) are being requested and a written solicitation will not be issued. Solicitation documents and incorporated provisions and clauses are those in effect through federal acquisition circular 97-23. This procurement is unrestricted. NAICS code 339112. Prospective vendors are required to submit written proposal for Canon CR-6/D200 digital imaging system, to include; Intel P-III 500 MMX Pentium Processor, computer with 600MHz, 128MB Ram, PCI Digital Imaging board, Dual 30 GB hard drives (1 primary and one removable for backup), 40 X C/D ratio Rom, 1.44mb 3 " floppy disc drive, Microsoft window software, 17" Color Monitor, 2-button mouse, Keyboard, AntiVirus software, and all accessories to include cabling and surge protector, (equal or better). Retinal digital imaging software Relational database, image enhancement (color balance, color channels, inversion), image proof sheet, image annotation, lossless file compression, zoom mode, C/D ratio, image formats, window 98, NT, and tele-medicine ready w/built-in E-mail, (equal or better). Cannon CR-6 non mydriatic retinal camera, Canon D2000 Digital Camera and adapter, Epson photo color printer, (equal/better) motorized table, and computer cart. Installation and training is required. This system/equipment will be used with the VISTA IMAGING. Vendor must provide confirmation that VA/DOD DICOM conformance statement has been approved by VA VISTA IMAGING PROJECT OFFICE and an interface with VISTA IMAGING has been written to ensure seamless integration with our electronic medical record. This is a firm-fixed price contract. Purchase only. This facility intends to purchase 3 units throughout FY01, with an initial delivery date of May 5, 2001. Offers shall be submitted f.o.b. destination. All offerors are to include a completed copy of the provisions at 52.212-3, Offeror Representations and Certifications Commercial Items, with offer, (a copy of this provision may be requested by fax to (210-617-5255). The following provisions apply to this acquisition: 52.212-1, Instruction to Offerors-Commercial Items, 52.212-2, Evaluation-Commercial Items, 52.212-4, Contract Terms and Conditions-Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, 52.211-6, Brand Name or Equal. The proposals will be evaluated on the following factors: 1) VA/DOD Dicom Compatible, 2) Past Performance, 3) Technical Software to support system is in non-proprietary file format, 4) Price.
