Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-22. This procurement is a total small business set-aside. The North American Industry Classification System (NAICS) code is 621999, All Other Miscellaneous Ambulatory Care. The small business size standard is $7.5 million average annual receipts for the past three years. Request For Proposals (RFP) 515-25-01, Oncologist Services. The Aleda E. Lutz VA Medical Center, 1500 Weiss Street, Saginaw, Michigan, has a requirement for an on-site oncologist to conduct an oncology clinic approximately twelve hours per week (one full day clinic and one half day clinic). Offerors are encouraged to submit quotes based on performance of the following procedures and estimates: 1. Initial Consultations-300 each; 2. Follow-up consultations-850 each; and 3. Bone Marrow aspirations and biopsies-30 each. The contractor shall provide a board certified oncologist to conduct scheduled clinics for patients with oncology/hematology diagnoses or related conditions. The oncology clinic will encompass performing initial consults and follow-up visits, diagnostic work up and treatment of patients with oncologic and/ or hemologic conditions as deemed medically appropriate by the oncologist/hematologist physician or as requested by the VA Medical Staff members. The contract physician will be required to observe all Medical Center policies and memorandums and Medical Center By-Laws and shall meet or exceed all related and applicable JCAHO Standards. The provision at 52.212-1, Instructions to Offerors Commercial (OCT 2000), applies to this acquisition. The provision at 52.212-2, Evaluation -- Commercial Items (MAR 2001), applies to this procurement. Evaluation criteria, in descending order of importance are, (1) Past Performance, (2) Competency, and (3) price. Past Performance and Competency are significantly more important than Price. Offerors are required to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, ALT III (FEB 2001), with its offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items (FEB 2001), applies to this acquisition with the following clauses added as addenda to the clause: 52.204-4 Printing/Copying Double Sided on Recycled Paper (AUG 2000), 52.224-1 Privacy Act Notification (APR 1984), 52.224-2 Privacy Act (APR 1984), 852.270-4 Commercial Advertising (NOV 1984), 852.237-70 Contractors Responsibilities (APR 1984), 852.270-1 Representatives of Contracting Officers (APR 1984), 52.217-8 Option to Extend Services (NOV 1999), 852.237-7 Indemnification and Medical Liability Insurance (OCT 1996).
