- Provide labor and material and supervision to clean exhaust and supply units #AH-19 and #AH-20 and duct work in VA Medical Center, Minneapolis, MN Operating Room. Mobilization and takedown/cleanup will be required for each unit as each unit will require stopping and starting as well as possible access points that need to be fabricated. Perform all work on weekends (or after 5:00 p.m. Friday and before 7:00 a.m. Monday). Vendor must be a member in good standing, qualified and certified as an air system cleaning specialist by National Air Duct Cleaners Association (NADCA) or equal organization. Perform all work in accordance with Request for Quotation RFQ 618-131-01, document available on or before 4/6/01. Service Contract Act applies -- Wage Determination #94-2287 (Rev. 15) dated 9/15/01, "Mechanics and Maintenance and Repair Occupations" applies to resulting agreement. Offeror must complete representations and certifications. A Technical Evaluation Panel will evaluate all offers for best value to the government, ranking offers on past performance and location, reputation in the industry (supply references), technical response to the RFQ, compliance with timelines, and Price and Price related factors. Vendor may submit multiple offers suggesting various ways to conduct the labor within the parameters required. Procurement is 100% small business set aside under NAICS code #541330, size standard of $4 million. Request RFQ 618-131-01 solicitation and drawings in writing or by fax 612-725-2072 on or before 4/6/01; a site walk is scheduled for early morning, Thursday, April 19, 2001 at 7:00 a.m. Meet in Facilities Engineering Service, Room BR-140 of Bldg. 70, the medical center.
