Description
The U.S. Army Soldier Biological and Chemical Command located at Natick, MA wishes to procure, through full and open competition utilizing FAR Part 12 Commercial Item procedures, quantities of three types of search mirrors, additional lenses, and lens assemblies. These commercial items may require minor modifications to meet government requirements. The government wishes to procure a total quantity of 1,916 search mirrors in the following types and quantities: 498 Type I Search Mirrors, Large including a convex lens and with no arm rest (Pricing Item 0001); 498 Type II Search Mirrors, Large including a convex lens with an arm rest (Pricing Item 0002); 920 Type III Search Mirror, Small with either a convex or flat mirror (Pricing Item 0003). In addition to the three types of search mirrors, the government wishes to purchase a quantity of 76 lens assemblies (Pricing Item 0004) for the convex mirrors and a quantity of 76 Lenses (only) (Pricing Item 0005) for the convex mirrors for spare parts. All three types of mirrors shall meet the following performance requirements: polished mirror surfaces capable of forming reflective images and resistant to scratches/abrasions to maximum extent possible and shock resistant so that the reflective face will not break during routine use; and storage and operating capability so that mirrors shall not degrade when stored and operated in climatic design types of hot, basic, and cold IAW Army Regulation 70-38 ("http://www.usace.army.mil/inet/usace-docs/army-reg/ar70-38"), and shall be operable by the central 90 percentile of male and female user population. The Type I and II search mirrors shall have a Mirror View Surface between 50 to 113 square inches; length shall be sufficient to search under a 5-ton vehicle, from sides of vehicle, by folding or extending mirror. A Flashlight Clip/Bracket is desired (but not required) for a standard Mag-type flashlight attachment. The Type III Small Tactical Mirror shall have a Size, Weight, and Mirror View Surface compact enough to fit BDU cargo pocket, with an 8" X 8" maximum view surface size with a desired weight of 8 ounces or less, and with a maximum weight requirement not to exceed 16 ounces. The required delivery of the quantities of search mirrors and spare parts is 90 calendar days after award of a purchase order and shall be shipped F.O.B. Destination to the following CONUS and OCONUS locations: Fort Bragg, NC (two ship to addresses) (140 Type I, 140 Type II, 230 Type III, 16 Lens, and 16 Lens Assemblies); Fort Hood, TX (50 Type I, 50 Type II, 50 Type III, 4 Lens, and 4 Lens Assemblies); Fort Lewis, WA (two ship to addresses) (75 Type I, 75 Type II, 100 Type III, 8 Lens, and 8 Lens Assemblies); Fort Benning, GA (30 Type I, 30 Type II, 60 Type III, 6 Lens, and 6 Lens Assemblies); Fort Polk, LA (15 Type I, 15 Type II, 30 Type III, 4 Lens, and 4 Lens Assemblies); Fort Leonardwood, MO (6 Type I, 6 Type II, 60 Type III, 2 Lens, and 2 Lens Assemblies); Fort Leavenworth, KS (3 Type I, 3 Type II, 50 Type III, 2 Lens, and 2 Lens Assemblies); Fort Sill, OK (3 Type I, 3 Type II, 20 Type III, 2 Lens, and 2 Lens Assemblies); Fort Knox, KY (3 Type I, 3 Type II, 20 Type III, 2 Lens, and 2 Lens Assemblies); Fort Shafter, HI (15 Type I, 15 Type II, 25 Type III, 4 Lens, and 4 Lens Assemblies); Schofield Barracks, HI (15 Type I, 15 Type II, 25 Type III, 4 Lens, and 4 Lens Assemblies); Fort Richardson, AK (3 Type I, 3 Type II, 25 Type III, 2 Lens, and 2 Lens Assemblies); Mannheim, GE (two ship to addresses) (50 Type I, 50 Type II, 60 Type III, 8 Lens, and 8 Lens Assemblies); Vicenza, IT (15 Type I, 15 Type II, 30 Type III, 4 Lens, and 4 Lens Assemblies); and Eight US Army Korea, APO AP 96205-0047 (75 Type I, 75 Type II, 135 Type III, 8 Lens, and 8 Lens Assemblies). All items shall be preserved, packaged, and shipped F.O.B. Destination in accordance with best commercial practices to the above locations. Complete addresses with DODACC, point of contact, and telephone numbers will be contained in any purchase order resulting from this combined synopsis/ solicitation. This synopsis/solicitation is prepared in accordance with the format in FAR subpart 12.6 and supplemented with additional information included below in this notice. This announcement will constitute the only solicitation. A request for a hard copy of this RFQ will not be honored. Vendors are required to submit a hard copy quotation and product demonstration samples (one of each type of Search Mirror) as described herein. The following clauses or provisions are specifically references and are required in the quoter's response to this combined synopsis/solicitation: 52.212-1 Instructions to Offerors -- Commercial Items; 52.212-2 Evaluation -- Commercial Items; 52.212-3 Offeror Representation and Certifications -- Commercial Items (the quoter must download the reps/certs from the website below, fill in the requested information, and submit those completed reps/certs with the quotation); 52.212-4 Contract Terms and Conditions -- Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items; 252.225-7012 Preference for Certain Domestic Commodities; and 252-225-7014 Preference for Domestic Specialty Metals. All clauses applicable to this solicitation may be accessed electronically at the following addresses: "http://www.arnet.gov/far; http://farsite.hill.af.mil; and http://www.dtic.mil/dfars". The following evaluation criteria, in descending order of importance, will be used to evaluate offeror's quotation submissions and product demonstration samples: Technical, Price, and Past Performance. The Technical Area is greater in importance than the Price Area which is slightly more important than the Past Performance Area. All evaluation areas other than price, when combined, are significantly more important than price. The Technical Area will be evaluated for (1) Written Proposal and Product Literature and (2) Product Demonstration Samples (one of each type of search mirror). Technical Subfactors (1) and (2) above are of equal importance. The Technical Area will be evaluated for the offered items' capability to meet all solicitation requirements as demonstrated by the written proposal, product literature, and user evaluation of the product demonstration samples (one Type I Search Mirror, Large, with no armrest; one Type II Search Mirror, Large, with armrest, and one Type III Search Mirror, Small). As part of the evaluation process, prior to award of a purchase order, the Type I and Type II items shall be field tested and evaluated for the following criteria: View Surface, Extendible or Retractable in Length, as needed, and for the Desired Attribute of a Flashlight Bracket. The Type III tactical mirror will be field tested and evaluated for its Size, Weight, and View Surface. It is anticipated that one purchase order will be awarded as a result of this procurement. Each offeror for this Request For Quotation shall submit a Product Demonstration Sample (PDS) of EACH type of mirror described above; a written proposal and product literature for each type of search mirror submitted for evaluation. The quoter shall also submit F.O.B. Destination pricing for each of the five items (Item 0001 Type I Search Mirror; Item 0002 Type II Search Mirror; Item 0003 Type III Search Mirror; Item 0004 Lens Assemblies; and Item 0005 Lenses) based on the delivery addresses delineated above. Price will be evaluated for price realism, reasonableness and fairness based on the competitive pricing received as a result of this acquisition. Lastly, the quoter shall submit three recent, relevant references demonstrating experience in providing these or similar items. The Government will use this information to evaluate the quoter's past performance risk in delivering a quality and timely product. Past Performance information shall contain the company name, address, current point of contact, telephone number or current e-mail address. Request for Quotation submission shall consist of two volumes. Technical information and Past Performance information shall be contained in Volume I of the quotation submission. The Technical information shall not exceed 30 pages and shall describe how their item(s) meet the requirements stated herein. The 30-page limitation includes product literature for all three search mirrors. Vendors shall submit written past performance information not exceeding two pages containing the information identified above. Pricing and representations and certifications shall be contained in Volume II of the quotation submission. Please submit an original and two copies of each volume. Quotations shall be submitted to: U. S.
