Description

Contractor shall provide Occupational and Physical Therapy Registry Services for the Department of Veterans Affairs Medical Center, 4150 Clement Street, San Francisco, CA 94121 and the DVA Northern California Health Care System, 150 Muir Road, Martinez, CA 94553. The Contract is for a base year and four -one year options to renew. The contract is to temporarily replace DVA employees when on extended sick leave or while hiring FTEE and other causes of shortages. This is a commercial items contract and is non restrictive. The contract may be awarded to multiple offerors. The following clauses and provision plus an addendum apply: 52.212-1 Instructions to Offorors -- Commercial Items, 52.212-2 Evaluations -- Commercial Items, 52.212-3 Representations and Certifications -- Commercial Items, 52.212-4 Terms and Conditions -- Commercial Items, 52.212-5 Terms and Conditions to Implement Statutes and Executive Orders -- Commercial Items.
