Description

Sierra Army Health Clinic, Herlong, CA, intends to establish a firm fixed-priced contract to provide for Occupational Health Services to include the development, maintaining and implementation of an Occupational Medicine Program, job related medical surveillance examinations and other evaluations/oversight as required per the applicable regulations. Subject solicitation is for commercial items and is being processed in accordance with the format in FAR Subpart 12.6. It is being issued as a Request for Proposals (RFP) using Simplified Acquisition Procedures (SAP) authorized in FAR Subpart 13.5, Testing for Certain Commercial Items. The solicitation number is DADA13-01-R-0011, the NAICS code is 926150 and the size standard is $5.0 million. The contract will be established for a base period plus four one year option periods, beginning on or about 01 May 2001 through 30 September 2001 for the base period. This requirement is for on, ten-hour day per week at the Sierra Army Health Clinic.
