- Description
- This requisition is for Backflow Identification. Provide all equipment and personnel to Locate, Identify, Test, and Certify all Backflow Preventors at Fort Lewis, Washington that are not currently under contract. Provide a list of building numbers where Backflow Preventors are located. The electronic data program must be compatible with Maximo/Microsoft Access latest version. Provide a one line drawing consisting of location within building structures or building perimeter of Backflow Preventors. Type and function shall identify Backflow Preventors. Estimated quantities are 700 devices. The contractor will identify the following as applicable: Building number, facility, location, manufacturer, size, model, type, and serial number. A sixty-day time period is allotted for completion of this contract; contract begins on the award date signed by the contracting officer. All Backflow devices that need repair or replacement and are not up to code will be identified, and contractor shall describe their individual deficiencies in the report, along with Backflow devices that serve no purpose. Provide a schedule of work for approval. Normal working hours: 0700 to 1700 Monday through Friday unless previously arranged and approved by the contracting officer. If work will require that the contractor work other than normal working hours as stated above; the contractor may request, in writing to the Contracting Officer, to work other than normal hours. Highway vehicles owned or leased by contractor shall be furnished with identifying markings reflecting at a minimum; the contractor's name, home city and local phone number. Contractor's workmen shall have on either a uniform with the Firm name and the workman's last name or shall have a badge Pinned on with both the firm name and the workman's photograph and full name. All buildings and wash racks on Main Fort Lewis and North Fort Lewis will be surveyed for Backflow Preventors, except as noted. Buildings exempt from the survey are the following: Madigan Hospital, and support buildings in the Madigan area, all of family housing areas, World War II wooden barracks, and World War II administration buildings on North Fort Lewis and in the Main Post areas. The following will be provided upon request. 1). A list of buildings and backflow preventors already under contract. 2). The Public Works Environmental Standard. This standard states in part that the contractor shall be familiar with, and incorporate into this work, the requirements outlined in the Fort Lewis Environmental Policy and the Public Works Environmental Management System (EMS). The EMS is based on the ISO 14000 standard (Environmental Management System specification with guidance for use) and its purpose is to ensure that all activities, procedures, and processes on the installation conform to the Fort Lewis Environmental Program. The contractor shall incorporate procedures to ensure that the work does not produce negative impacts to the environment and ensure that all submittals are produced using recycled paper and electronically. This requirement is 100 percent set-aside as small business/small purchase. The size standard for a small business concern including its affiliates is a maximum of 500 employees. NAICS Code #333911. All faxed requests must include an email address. The solicitation will be issued and available for download at the Fort Lewis Contracting Office Web site located at http://www.lewis.army.mil/doc on the issue date of the solicitation. The anticipated issue date for this solicitation is May 9, 2001.
