- PURCHASE AND INSTALLATION OF AFCA PROJECTORS. -- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. The procurement is to furnish all labor, equipment, tools, materials, electricity, transportation, and supervision necessary to provide removal of Barco Projectors and installation of LCD Projectors in the AFCA Seminar conference rooms at Scott AFB IL in accordance with the attached Statement of Work (SOW). Inspection and acceptance will be at Scott AFB IL. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. This solicitation, reference number F1380510940300, is being issued as a Request for Quotation (RFQ). This solicitation is set-aside 100% for small business. The North American Industry Classification System Code (NAICS) is 333315 and the small business size standard is 500 employees. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-19. Vendor must be actively registered with Central Contractor Registration. Offerors must comply with all instructions contained in FAR 52.212-1, Instructions to Offerors-Commercial. FAR clause 52.212-2, Evaluation-Commercial Item applies. Award of this acquisition will be based on Best Value to the Government and the evaluation criteria for award will be technical proposal, past performance and price. If proposal is technically acceptable, past performance will be evaluated as significantly more important than price. Offerors will provide a list of three (3) of the most relevant (like or similar) services performed for either the Federal Government or commercial customers within the last three years. Past Performance references must be submitted along with price quote. The following additional FAR provisions and clauses do apply: FAR 52.212-3 Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer. STATEMENT OF WORK FOR MAINTENANCE AND REPAIR AIR FORCE COMMUNICATIONS AGENCY VI UPGRADE PLAN _ SEMINARS PROJECTORS 4 Apr 01 1. SCOPE The intent of this Statement of Work is to provide replacement of the Barco CRT projectors in the Seminars conference rooms. This upgrade plan is in accordance with a site survey accomplished by SAIC in Nov 99. Should it be found that the standards herein specified are not being satisfactorily accomplished, the government may immediately demand the contractor take action to ensure the equipment meets these requirements. 2. REQUIREMENT a. The contractor shall perform installation (to include mounting hardware and focal lens requirements), connectivity, adjustments and operational tests of the LCD Projector (reference Section 1) in the AFCA Seminars conference room (4 each). Also included is de-installation of the current Barco projectors (4 each). The contractor will move the Barcos to a designated location by the QAE. The contractor will perform this work at an agreed time schedule with the QAE. The contractor will perform a site survey to ensure all requirements are met. The Seminars projection room requires mirrored projection equipment (GOE) and a short focal distance. b. The contractor shall establish with AFCA/CCQI VI, who is designated a Quality Assurance Evaluator (QAE), authority to inspect and accept or reject the results of the contractor_s installation. c. The contractor shall provide all labor, and transportation to perform the following services: install, connect, adjust and operationally test the installed equipment. d. Installation service shall be accomplished by certified authorized dealer personnel for the purpose of keeping property in efficient condition. As a minimum, all work shall be accomplished in accordance with the applicable sections of the manufacturer's maintenance and operations manuals. e. The contractor shall install to the extent necessary to ensure a safe and serviceable equipment item or system that shall efficiently serve its intended purpose as determined by inspection, test or disassembly as required. f. The contractor shall ensure materials or parts supplied be new and adhere to the original equipment manufacturer's specifications. Extend to the government all commercial warranties commonly associated with the new parts and consistent with the standard commercial practices. Written warranties when available on such parts shall be furnished to the QAE. g. The contractor shall make no changes to the existing circuits, wiring design, or otherwise alter the original circuit or wiring design of any associated equipment items. Ensure the original design and functional capabilities shall not be changed, modified or altered unless such changes are authorized by QAE. e. The contractor shall have available, suitable, modern equipment necessary for the satisfactory execution of the installation and all special tools for the proper servicing of the equipment item or system called for under this contract. f. The contractor shall ensure that every safety precaution has been taken prior to performing any service under this contract, and that the work areas are properly cleared of all contractor's residue upon completion of service. 3. GENERAL: a. Time and place of performance: Services shall be accomplished at the Air Force Communications Agency (AFCA) Building 1700, 203 W. Losey St., Scott Air Force Base, Illinois during the normal duty hours of 0700 to 1700, and normal duty days of Monday to Friday excluding legal federal holidays. b. In the event the government determines a deliverable is unacceptable by the contracting officer, or the contractor is not meeting the approved schedules, the contractor will be notified in writing within one working day. The written deficiency notice will specify the nature of such deficiency and delineate contractor required remedial actions. The contractor shall be given three working days to rectify the specified deficiency to the satisfaction of the government. c.
