Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number DABT31-01-T-0068 for the acquisition of Low Back Chairs and Training Tables is issued as a request for quotation. Solicitation document, incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-15. RFQ is a small business set-aside. Sic is 2531; size standard is 500 employees. Closing date is 5 May 2001, 12:00 p.m. CST. POC for additional information is Robert Ebaugh, Contract Specialist, (573)596-0261 or Donald F. Blotzer, Contracting Officer, (573)596-0251. The following provisions and clauses apply to this RFQ: 52.212-1, 52.212-3, 42.212-4, 52.212-5 (Dev), 52.233-33, DFAR 252.212-7001 (Dev), 252.204-7004. Description of the requirement is as follows: 0001, Chair, Low Back with adjustable T-arms, or equal to UNICOR Style; Legacy UST-2006-BLK1111, Some of standard features include but not limited to are; swivel tilt mechanism with variable position back lock, seat depth adjustment, back height adjustment and 6 degree forward tilt, tension control, 5 1/2" pneumatic seat height adjustment, self-adjusting lumbar back support, Enersorb seat foam for excellent back and pelvic support, vertical and horizontal adjustable arms with tear drop polyurethane arm pads. Casters capable for use on floors such as carpeting. Cloth is Cranberry with Trim color in black. Quantity is (80) each. 0002, Table, Training, Heavy Duty, with such features two 14 gauge square tube C-legs with adjustable glides, a 16 gauge modesty panel and laminate Or equal. Measurements are 24" x 60" x 28 1/2". Quantity is (40) each. All items are to be destination priced and delivered to Fort Leonard Wood, Missouri 65473 by 23 May 2001. Submitted proposals shall be on Company letterhead signed by an authorized individual and include; prices, delivery date, manufacturers literature to include specifications, Federal Tax Identification Number.
