Description

This is an amendment to combined synopsis/solicitation RFP 658-32-01 originally posted in the CBD April 11, 2001 for PHARMACY SERVICES for VAMC Salem, Virginia. This amendment adds an additional Community Based Outpatient Clinic (CBOC) located at Martinsville Family Medical Center, 315 Hospital Drive, Suite 101, Martinsville, Virginia 24112 to the original synopsis/solicitation. The Government reserves the right to make multiple awards from this synopsis/solicitation. Proposed prices, on company letterhead, completed Reps and Certs, FAR 52.212-3 Offeror Representations and Certifications Commercial Items, and VAAR 852.219-70 Veteran-Owned Small Business may be submitted by fax to Contracting Officer (90C3) at (540) 983-1065. Copies with original signatures shall be submitted via mail or hand carried to Contracting Officer (90C3), VA Medical Center, 1970 Roanoke Boulevard, Salem, Virginia 24153. Due date for proposals remains close of business, 4:30 p.m., local time (EST), Monday, May 14, 2001. All terms and conditions of original synopsis/solicitation shall apply.
