Description

Synopsis: The Great Lakes Acquisition Center (GLAC) is soliciting quotations to furnish Statement of Conditions Survey for the 2000 JCAHO cycle for all eight (8) Medical Centers in VISN 12 (VA Chicago HCS, Lakeside & West Side Division, Chicago, IL., Edward Hines VA, Hines, IL.; VAMC Iron Mountain, Iron Mountain, MI.; VAMC Tomah, Tomah, WI.; WM S Middleton VA Hospital, Madison, WI.; & VAMC Milwaukee, Milwaukee, WI.) The start date is estimated to be June 1, 2001 with a completion date of September 30 2001. This Contract is a firm-fixed price. This solicitation is a small business set-aside. Hard copy and electronic versions of the Solicitation will be available on or about May 3, 2001. Requests for hard copy solicitation must be in writing to the Contracting Officer at the GLAC and accompanied by a check for $5.00 made out to the Department of Veterans Affairs.
