Description

The Federal Correctional Institution, F.C.I. Petersburg is seeking interested parties that might be interested in receiving our solicitation for Optometrist Services to be provided at F.C.I. Petersburg Low and Medium Institutions. The Contractor shall provide 4 sessions per month with a session defines as 4 hours in length. 2 each 4 hour session a month will be provided for F.C.I. Petersburg Low and 2 each 4 hours session a month will be provided for F.C.I. Petersburg Medium. (This is a total of 48 sessions per year.) The Contractor shall provide examination, diagnoses, and make recommendations to the Clinical Director for appropriate optometric care/further treatment of ocular disease's, prescribe eyewear, and any medically indicated features of eyewear for patient's unique ocular needs. The Contractor shall also perform tonometries and slit lamp examinations when medically indicated/requested, and make necessary adjustments of institution glasses/frames dispensed. The Contractor shall make recommendations to the Health Service Administrator and Clinical Director for treatment of eye diseases and the procurement of eyewear. The Contractor shall have a degree from a credited college or university, and hold a current state license that permits them to practice Optometry in the State of Virginia, and shall maintain their own liability insurance from a responsible insurance carrier in an amount of not less than $1,000,000.00. This procurement will be set aside for small business and will be a Firm-Fixed Price Requirements Type Contract. It is anticipated this contract will not exceed $25,000.00. The contract shall consist of a Base Year, with four (4) one year option periods. The solicitation will be issued on or about August 1, 2001 and all bids should be submitted by September 4, 2001.
